摘 要:目的探讨妊高征患者红细胞内外的Ca2+、Mg2+和环磷酸腺苷(cAMP)、环磷酸鸟苷(CGMP)的含量变化、相互关 系及与妊高征发病机制的关系。方法采用生化方法测定25例妊高征患者和27例正常晚期妊娠妇女红细胞膜ATP酶 活性、红细胞内外 Ca2+、Mg2+含量;采用竞争性蛋白质结合分析方法测定血浆 cAMP、放射免疫方法测定 cGMP。结果 妊高征组红细胞膜Na+-、K+-ATP酶和Ca2+-ATP酶活性明显降低(P<0.001),而Mg2+-ATP酶活性未见改变。红细胞内 Ca2+含量显著增高(P<0.01),而Mg2+含量明显降低(P<0.01)。血浆Ca2+、Mg2+的浓度显著低于对照组(P<0.05),而血浆 cAMP和 cGMP的浓度显著增高 (P<0.05)。妊高征组的平均动脉压(MAP)与红细胞膜的 Ca2+-、 Na+-、K+-ATPase的活性 呈负相关,与Ca2+、cAMP和cGMP呈正相关。Ca2+-ATP酶与Ca2+呈负相关;Ca2+与cMAP和cGMP呈正相关。结论在 妊高征的发生机制中缺镁比缺钙更有意义。细胞Ca2+转运障碍和cAMP、cGMP的代谢异常可能与妊高征的发生有关,Objective To study the changes in the levels of intraerythrocytic and extraerythrocytic Ca2+.Mg2+ and plasma cAMP.cGMP in patients with pregnancy-induced hypertension (PIH) before and after delivery, and to investigate the interaction between the above factors and their role in the pathogenesis of PIH. Methods Twenty-five patients with PIH and 27 healthy women in fare pregnancy serving as normal control were enrolled in this study. The activity of ATPase and the levels of the intraerythrocytic, extraerythrocytic and plasma Ca2+.M2g+ in all the subjects were measured with biochemical method. The level of the plasma cAMP was determined by competitive protein binding assay and the plasma eGMP level by radioimmunal assay (RIA). The correlations of the above factors with the mean arterial blood pressure (MAP) were determined. Results The activity of Na+-, K+- and Ca2+-ATPase were significantly lower in PIH group than those in the control group (P<0. 001), whereas Mg2+-ATPase did not statistically differ between the 2 groups. In PIH group, the concentration of intraerythrocytic Ca2+ was significantly higher (P<0.01), while that of Mg2+ showed contrary result. Compared with the normal controls, the concentrations of plasma Ca2+ and Mg2+ were lower, while those of plasma cAMP, and cGMP were higher in PIH group. In PIH group, the MAP is negatively correlated with the activity of Na+-, K+- and Ca2+-ATPase and positively with the intraerythrocytic Ca2+, plasma cAMP and cGMP. The activity of Ca2+-ATPase was negatively correlated with the intraerythrocytic Ca2+, which in trun showed positive correlation with plasma cAMP, and cGMP. Conclusions ~Magnesium deficiency plays a more important role in the pathogenesis of PIH than calcium deficiency does. The transportation disturbance of Ca2+ and metabolic abnormality of cyclic nucleotide might be associated with the occurrence of PIH.
