脑瘫患儿七氟醚吸入诱导喉罩置入最佳时机BIS值探讨

摘　　要：目的 探讨脑瘫患儿七氟醚吸入诱导喉罩置入最佳时机的脑电双频指数（BIS）值.方法 35例择期行肌力、肌张力调整术的脑瘫患儿,ASA I～Ⅱ级,年龄2～12岁,性别不限,体重10～48Kg.采用七氟醚吸入诱导,按照序贯试验的方法设定每例患儿可进行喉罩置入的BIS值,计算喉罩置入的BIS50及其95%可信区间（CI）.结果 随着BIS值的降低,喉罩置入的满意率升高（P＜0.01）.小儿七氟醚吸入诱导下置入喉罩的BIS50为50.35,95%可信区间（CI）为（47.38～53.51）.结论 脑瘫患儿七氟醚吸入诱导时,根据BIS值判定喉罩置入时机可提高喉罩置入的成功率.BIS值为44～48是置入喉罩的适宜时机.Objective To investigate the value of bispectral index（B[S） in the best time for laryngeal mask airway （LMA） insertion during sevoflurane inhalation induction in children with cerebral palsy surgery. Methods Thirty five children with cerebral palsy ASA I or II of both sexes, aged 2 - 12 year with weighing 10 -48kg undergoing cerebral palsy muscle strength muscle tension adjustment method were inducted by sevoflurane inhalation, and set a BIS value for the feasibility of LMA insertion for each patient accord to the method of sequential test. The BIS50 and 95% confidence interval （CI） were calculated. Results The satisfaction rate of LMA insertion was increased with the lower BIS value. The BIS50 was 50. 35 and 95% confidence interval （CI） was from 47.38 to 53.51. Conclusion It can improve the success rate of LMA insertion according to the BIS value to judge the best time of LMA insertion. The BIS value of 44 - 48 is the appropriate opportunity of the LMA insertion.
