摘 要:目的回顾性分析2007~2011年,深圳地区儿童主要食物过敏原的变化情况,为儿童食物过敏的防治提供参考。方法对2007~2011年于深圳市儿童医院就诊的2986例疑似过敏患儿,采用体外过敏原检测系统首先进行混合食物过敏原筛查检测,部分阳性患儿进行了6种常见食物(鸡蛋白、牛奶、鱼、小麦、花生、大豆)的特异性IgE(sIgE)检测,并对5年间儿童食物过敏原分布的变化进行统计分析。结果5年间混合食物过敏原筛查总阳性率为37.0%(1104/2986),其中42.4%(468/1104)的阳性筛查标本进行了6种常见食物过敏原特异性IgE(sIgE)检测。5年间深圳地区食物过敏患儿的主要过敏原为鸡蛋白和牛奶,在进行了6种常见食物过敏原sIgE检测的患儿中,鸡蛋白slgE阳性率在62.5%~72.2%之间,牛奶slgE阳性率为56.9%~76.5%,仅2009年牛奶slgE阳性率较2008年显著上升(P〈O.05),5年间其他时间2种过敏原MgE阳性率均无明显变化。5年间鸡蛋白和牛奶slgE同时为阳性的比例在65%~73%间,5年间无明显变化。5年间,鱼slgE的阳性率最低,均在4%以下,无明显变化;小麦slgE阳性率在2007~2010年呈逐年上升的趋势,2011年回到正常水平,总体无显著变化;花生和大豆slgE阳性率5年间的变化规律性不显著,总体阳性率在5%~11%之间。结论深圳地区儿童最为主要的食物过敏原为鸡蛋白和牛奶,小麦、花生和大豆也是重要的过敏原,过敏原为鱼的比例较低。5年间深圳地区常见食物过敏原的分布较为稳定。Objective Retrospective analysis of children's common food allergens changes in Shenzhen city from 2007 to 2011 ,to provide clinical reference for prevention and treatment of food allergies in children. Methods A total of 2 986 children were suspec- ted of allergic diseases in Shenzhen Children's Hospital from 2007 to 2011, slgE were detected using Immuno CAP 100 (Phadia AB,Sweden). The mixed food allergen(fx5E)were firstly detected,then fx5E positive patients were detected in 6 common food al- lergens. 5-year changes in children's common food allergens were statistically analyzed. Results The total fx5E positive rate was 37.0% (1 104 cases) in 5 years,of whom 42.4% (468 cases) were found six common food allergen specific IgE (slgE). Egg white and cow milk were the major food allergens in Shenzhen children,egg white positive rate was 62.5%-72.2% ,and cow milk posi- tive rate was 56.9 %--76.5 %. Fish positive rate of lower than 4 %was the lowest in 5 years. Conclusion Egg white and cow milk were the major food allergens in Shenzhen children,positive rates are higher than 60%, fish has the lowest positive rate of 4%. Common food allergens are in a relatively stable level in Shenzhen.
