慢性阻塞性肺疾病患者治疗前后血浆基质金属蛋白酶-9及其抑制物-1水平的变化

摘　　要：目的 探讨COPD急性加重期患者治疗前后血浆基质金属蛋白酶-9（MMP-9）及抑制物1（TIMP-1）水平的变化.方法 选择COPD急性加重期患者50例（病例组）,予以吸氧、抗感染、止咳化痰及解痉平喘等常规治疗1周,观察治疗前后血浆MMP-9、TIMP-1水平和MMP-9/TIMP-1比值的变化,观察临床疗效.另选择同期健康体检者30例作为对照组.结果 病例组治疗前血浆MMP-9、TIMP-1水平和MMP-9/TIMP-1比值明显高于对照组（t=2.97、2.44、2.31,P＜0.05）.治疗1周后,病例组血浆TMMP-9、TIMP-1水平和MMP-9/TIMP-1比值较治疗前明显下降（t=2.86、2.39、2.27,P＜0.05）；治疗后临床总有效率为92.0%（46/50）.结论 COPD急性加重期患者血浆MMP-9、TIMP-1水平和MMP-9/TIMP-1比值升高,血浆MMP-9、TIMP-1水平和MMP-9/TIMP-1比值可作为COPD患者病情严重程度和治疗疗效观察指标.Objective To explore the changes of matrix metalloproteinase-9（MMP-9） and tissue inhibitor of metalloproteinase-1 （TIMP-1） in plasma before and after medical treatment of chronic obstructive pulmonary disease （COPD） patients.Methods 50 COPD patients in stage of attack were selected as case group,and were given routine medical treatment for 1 week,such as oxygen uptake,anti-infection,relieving cough and reducing sputum,spasmolysis and relieving asthma,and etc.The changes of MMP-9,TIMP-1 and MMP-9/TIMP-1 ratio levels before and after medical treatment were observed,and the clinical curative effect observation was carried on as well.Additionally,30 cases of normal personnels who were taking the physical examination at the same time were selected as the control group.Results The MMP-9,TIMP-1 and MMP-9/TIMP-1 ratio levels of patients in case group before medical treatment were much higher than those in control group （t =2.97,2.44,2.31,all P 〈 0.05）.After 1 week＆#39;s medical treatment,the MMP-9,TIMP-1 and MMP-9/TIMP-1 ratio levels of patients in case group declined obviously than before（t =2.86,2.39,2.27,all P 〈 0.05）,and the total clinical efficiency reached 92.0% （46/50）.Conclusion The MMP-9,TIMP-1 and MMP-9/TIMP-1 ratio levels in plasma of COPD patients are abnormally high,which can be the observation index of the severity of the disease and curative effect of COPD patients.
