摘 要:目的:探讨氧化低密度脂蛋白、一氧化氮(NO)对血管内皮功能及肾动脉血流动力学的影响,研究肾脏血流动力学改变在妊娠期高血压疾病中的临床应用价值。方法:选择住院分娩妊娠期高血压疾病(HDCP)患者56例为研究组,正常孕妇20例为对照组,使用彩色多普勒超声监测两组孕妇肾脏叶间动脉血流参数,采用酶联免疫吸附法测定患者血浆中氧化低密度脂蛋白的浓度,采用硝酸还原法测定血清中NO水平。结果:①子痫前期组肾叶间动脉PSV、EDV均低于对照组,差异有统计学意义(P<0.05);子痫前期组PSV与EDV比值、RI、PI均高于对照组,差异有统计学意义(P<0.05),妊娠期高血压组与对照组差异无统计学意义(P>0.05)。②研究组氧化低密度脂蛋白高于对照组,差异有统计学意义(P<0.01);妊娠期高血压组低于子痫前期组,差异有统计学意义(P<0.01)。③研究组NO浓度低于对照组,差异有统计学意义(P<0.01);子痫前期组NO低于高血压组,差异有统计学意义(P<0.01)。④研究组肾叶间动脉S/D、RI、PI与oxLDL成正相关(P<0.01),与NO成负相关(P<0.01),oxLDL与NO成负相关(P<0.01)。结论:肾动脉血流参数能够客观反映血管舒缩功能状态,对子痫前期的预测具有重要的临床价值。Objective: To explore the effects of oxidized low density lipoprotein and nitric oxide (NO) on vascular endothelial function and renal hemodynamics, research the clinical application value of renal hemodynamics in hypertensive disorder complicating preg nancy (HDCP) . Methods: Fifty HDCP patients and twenty normal pregnant women who gave birth to their babies in the hospital were se lected as study group and control group, respectively; color ultrasonography was used to detect the blood flow parameters of renal interlobar arteries in the two groups, the concentrations of serum oxidized low density lipoprotein in the two groups were measured by ELISA. The ser um levels of NO in the two groups were detected by nitric acid recovery method. Results : The levels of PSV and EDV in preeclampsia group were statistically significantly lower than those in control group ( P 〈 0. 05 ) . The levels of S/D, RI, and PI in preeclampsia group were sta tistically significantly higher than those in control group ( P 〈 0.05 ), but there was no statistically significant difference between HDCP group and control group ( P 〉 0. 05 ) . The level of oxidized low density lipoprotein in study group was statistically significantly higher than that in control group ( P 〈 0. 01 ) ; the level of oxidized low density lipoprotein in HDCP group was statistically significantly lower than that in pre eelampsia group ( P 〈 0. 01 ) . The level of NO in study group was statistically significantly lower than that in control group ( P 〈 0.01 ) ; the level of NO in preeelampsia group was statistically significantly lower than that in HDCP group ( P 〈 0. 01 ) . In study group, S/D, RI, and PI were positively correlated with oxidized low density lipoprotein ( P 〈 0.01 ), but they were negatively correlated with NO, there was a negative correlation between oxidized low density lipoprotein and NO (P 〈 0.01 ) .
