摘　　要：通过对个案管理及个案管理模式的概念、内涵等的介绍,结合国外相关报道及国内现况,阐述了我国内陆地区实施个案管理模式的必要性及有效性,并探讨了我国推行个案管理需注重的关键环节,为国内医疗卫生系统发展新的护理模式提供借鉴。This paper introduces the concept and connotation of case management,explains the necessity and outcomes of delivering case management model in China,and analyzes the key points in the delivery of case manage- ment mode,which may provide reference for the development of new nursing models in Chinese health care system.
