不同促排卵方案在卵巢功能减退患者中的有效性观察

摘　　要：目的比较促性腺激素释放激素激动剂(GnRH-a)长方案和促性腺激素释放激素抑制剂(GnRH-A)方案对卵巢功能减退患者促排卵效果和临床妊娠的影响,为促排卵优选方案提供理论依据。方法选取符合标准的患者80例,随机分为观察组(GnRH-A方案)和对照组(GnRH-a长方案)各40例,比较二组治疗效果。结果两组患者hCG日血清雌二醇(E2)、黄体生成素(LH)及孕酮(P)水平比较,差异无统计学意义(P>0.05);对照组Gn用药天数及用药量明显多于观察组,差异有统计学意义(P<0.05)。观察组临床妊娠率高于对照组,差异有统计学意义(P<0.05);对照组取卵数及受精数低于观察组,差异有统计学意义(P<0.05)。结论 GnRH-A方案可提高患者临床妊娠率,可能是卵巢功能减退患者一种较好的促排卵治疗方法。Objective To compare the effects of gonadotropin releasing hormone agonist （GnRH-a） and gonadotropin releasing hormone inhibitor （GnRH-A） on the ovarian stimulation outcome and clinical pregnancy in hypovaria patients, and to provide theoretic foundations for optimizing method for ovarian stimulation. Methods Eighty eligible patients were selected and randomly divided into observation group and control group, 40 patients each; the observation group used GnRH-A, while the control group used GnRH-a, and the therapeutic effects of the two groups were compared. Results The differences in serum estradiol （E2）, luteinizing hormone （LH） and progesterone （P） levels between the patients of the two groups were statistically insignificant （P〉0.05）; but the medication duration and drug consumption of the patients of the control group were significantly more than those of the observation group, and the differences were statistically significant （P〈0.05）. The clinical pregnancy rate of the patients of the observation group was higher than that of the control group, and the difference was statistically significant （P〈0.05）; the taken egg number and fertilized egg number of the patients in the control group were Cower than those of the observation group, and the differences were statistically significant （P〈0.05）. Conclusion GnRH-A can increase the clinical pregnancy rate of the patients, and is a good ovarian stimulation method for hypovaria patients.
