全程康复护理对恶性骨肿瘤保肢治疗下肢功能的影响

摘　　要：目的探讨人工假体置换或异体骨植骨结合新辅助化疗方案治疗恶性骨肿瘤患者的康复护理要点。方法对36例接受人工假体置换或异体骨植骨结合新辅助化疗方案治疗恶性骨肿瘤患者,随机分为对照组(18例)和干预组(18例)。对照组给予常规康复护理,包括术前常规护理及康复指导、化疗护理、心理护理、术后常规康复护理;干预组在常规护理的基础上,采用全程康复护理模式进行护理。对两组患者术后1w、2w、1月下肢功能进行比较。结果干预组与对照组下肢功能在术后1w、2w、1月总体比较,差异有统计学意义(P<0.01);干预组下肢功能在术前、术后1w、术后2w、术后1月相比较,差异有统计学意义(P<0.01)。结论全程康复护理能有效帮助恶性骨肿瘤保肢治疗患者下肢功能的恢复。Objective To study the key point of nursing patients who have malignances of bone and undertaken combined neo-adjuvant chemotherapy with prosthetic replacement or self-bone transplant. Methods 36 patients with bone malignancies and undertaken combined neo-adjuvant chemotherapy with prosthetic replacement or self bone transplant were randomly assigned to control （18 cases） and intervention groups. Patients in control groups were given routine nursing care, including preoperative routine nursing and rehabilitation guidance nursing care post chemotherapy, psychological care, and the conventional rehabilitation nursing after operation. In intervention group, in addition to aforementioned routine nursing care, the whole course rehabilitation nursing was given. Function of lower extremity was compared between two groups at 1 week, 2 week and 1 month. Results In comparison with control group, patients in the whole course rehabilitation nursing group had significantly better lower extremity function at 1 week, 2 week and 1 month （P〈0. 0l）. In the intervention group, the function of lower extremity was significantly different in pre-treatment, 1 week, 2 week and 1 month post-operation （P〈0. 01）. Conclusion The whole course rehabilitation nursing could improve the reconstruction of lower extremity function.
