摘　　要：目的:介绍我院更年期综合门诊患者用药情况以及临床药师参与的用药管理模式。方法:分析就诊的255例患者疾病及用药情况,药师提供咨询、指导教育及档案建立等系统的用药管理服务。结果:患者为年龄(50.3±6.9)岁的更年期女性,药物治疗主要涵盖降糖药(6.8%)、性激素(10.1%)、调脂药(11.8%)、骨质疏松治疗药物(12.3%)、精神障碍治疗药物(14.7%)、降压药(20.3%)和植物药(26.1%)等。药师对255例患者进行用药管理,协助处置药品不良反应15例,对随访的38例重点患者进行全程监护和评估。结论:更年期患者用药复杂;临床药师参与用药管理提高了药物治疗的正确率、依从性,优化了临床获益。OBJECTIVE： To introduce the medication of menopausal patient and the medication management model in which clinical pharmacists involved in our hospital. METHODS ： The diseases and medications of 255 patients were analyzed, and pharma- cists provide systemic management as consultation, guidance education and file establishment. RESULTS： Patients were menopaus- al female aged （50.3 _＋ 6.9） years. Drugs mainly covered hypoglycemic agents （6.8%）, sex hormone （10.1%）, lipid-lowering drugs （11.8%）, drugs for osteoporosis （12.3%）, drugs for mental disorders （14.7%）, antihypertensive drugs （20.3%）, botani- cals （26.1%） and so on. Pharmacist provided medication management for 255 patients, disposed 15 cases of adverse drug reac- tions, and followed up 38 cases for care and evaluation. CONCLUSIONS： Menopausal patients have been given complex medication; pharmacist can improve the accuracy and compliance of drug therapy, and finally optimize clinical outcomes.
