摘 要:目的动态检测复发性鼻息肉组织及术后术区鼻黏膜组织血管内皮生长因子(VEGF)的变化规律,探讨鼻息肉复发与VEGF的关系。方法选取38例.复发性鼻息肉患者,采用鼻内镜手术干预,放射免疫分析法动态检测复发性鼻息肉患者鼻黏膜组织VEGF水平,总结其变化规律。结果复发性鼻息肉标本VEGF为(90±22)pg/ml,术后2周明显下降(16±4)pg/ml,术后4周VEGF为(154±20)pg/ml,术后8周VEGF(244±66)pg/ml达高峰,术后12周明显下降至(6±4)pg/ml。结论复发性鼻息肉鼻内镜术后2周VEGF水平明显降低,随后逐渐升高,术后8周达最高峰,术后12周明显下降,可达正常水平。放射免疫法动态测定VEGF可作为鼻息肉复发的重要参考指标,术后4—8周积极进行鼻内镜清理,局部应用糖皮质激素积极干预可以明显降低鼻息肉复发率。Objective To investigate the relationship between the recurrence of nasal polyps and VEGF in nasal polyps and nasal tissue in surgery area after operation. Methods Thirty-eight patients were treated under nasal endoscope. The nasal tissue VEGF levels of patients with recurrent nasal polyps were dynamicly detected by radioimmunoassay, and its variation regulation was summarized. Results The VEGF level of recurrent nasal polyps specimens was (90 ± 22)pg/ml, 2 weeks after operation decreased significantly by( 16 ± 4 ) pg/ml, 4 weeks later was ( 154 ± 20) pg/ml, 8 weeks after surgery was ( 244 ± 66) pg/ml and reached to peak, after 12 weeks decreased by(6 ±4)pg/ml. Conclusions VEGF levels decreased significantly two weeks after nasal endoscopy of recurrent nasal polyps, and then gradually in-creased, it reached to peak at 8 weeks after surgery. Twelve weeks after surgery, decreased to normal levels. Radioimmunoassay dynamic determination of VEGF can be used as an important reference index for predicting postoperative recurrent nasal polyps, 4-8 weeks after positive endoscopic cleaning, topical nasal glucocorticoids active intervention can significantly reduce nasal polyp recurrence.
