尖吻蝮蛇血凝酶用于子宫颈环形电切术治疗宫颈疾病止血效果观察

摘　　要：目的评价尖吻蝮蛇血凝酶在行子宫颈环形电切术(LEEP)治疗宫颈疾病中的止血效果。方法将62例行LEEP的子宫颈上皮内瘤变(CIN)患者随机分为尖吻蝮蛇血凝酶组(试验组)和氨甲环酸组(对照组),比较两组患者术中、术后出血量,手术时间、术后阴道流血时间及术后发热和腹痛发病率并评价止血效果。结果术中出血试验组为(25.2±3.3)mL,明显少于对照组的(38.4±4.4)mL(t=2.4,P<0.05);手术时间试验组为(7.3±0.7)min,明显少于对照组的(9.9±1.0)min(t=2.2,P<0.05);术后出血试验组为(73.9±5.7)mL,明显少于对照组的(98.7±8.5)mL(t=2.4,P<0.05)。两组术后阴道流血时间、术后发热和腹痛发病率比较,无明显差异(P>0.05)。结论尖吻蝮蛇血凝酶可作为一种较理想的新型止血药物,可用于LEEP治疗宫颈疾病的术中、术后止血。Objective To evaluate the hemostatic effect of haemocoagulase agkistrodon in cervical diseases treated by loop electrosurgical excision procedure （LEEP）.Methods Sixty-two patients with cervical intraepithelial neoplasia（CIN） treated by LEEP were randomly divided into the haemocoagulase agkistrodon group （experiment group） and the tranexamic acid group （control group）.Intraoperative bleeding,postoperative bleeding,operation time,postoperative vaginal bleeding time and the occurrence rate of postoperative fever and abdominal pain were compared between the two groups for evaluating the hemostatic effects of the two kinds of hemostatic drug.Results The intraoperative bleeding volume was （25.2 ±3.3）mL in the experiment group,which was significantly lower than （38.4 ±4.4）mL in the control group（t =2.4,P 〈0.05）; the operative time was （7.3 ± 0.7）min in the experiment group,which was significantly lower than （9.9-± 1.0） min in the control group（t =2.2,P 〈 0.05）; the postoperative bleeding volume was （73.9--5.7） mL in the experiment group,which was significantly lower than （98.7 ± 8.5）mL in the control group（t =2.4,P 〈 0.05）; there was no statistical difference in the postoperative vaginal bleeding time and the occurrence rate of postoperative fever and abdominal pain between the two groups（P〉0.05）.Conclusion Haemocoagulase agkistrodon is an ideal new type of hemostatic medicine in treating intraoperative and postoperative bleeding in cervical disease treated by LEEP.
