中老年人群尿酸与颈动脉粥样硬化相关性

摘　　要：目的：探讨中老年人群尿酸与颈动脉粥样硬化（CAS）相关性。方法选择358例2010年1月至2012年12月在体检中心查体的中老年人群，颈动脉超声测定颈动脉内中膜厚度（IMT），根据测量结果分CAS斑块组（n＝106）和无斑块组（n＝252）；按血尿酸水平的三分位数以及性别分为 A 组（男：血尿酸＜302μmol/L 女：＜268μmol/L）、B 组（男：血尿酸302～354μmol/L，女：268～312μmol/L）、C组（男：血尿酸＞354μmol/L，女：＞312μmol/L）。结果CAS斑块组血尿酸水平明显高于无斑块组（P＜0．05），随着血尿酸水平升高，CAS斑块的发生率及IMT亦相应增加（P＜0．05），其中C组CAS斑块的发生率是正常尿酸者的4倍。而经Logistic回归分析后显示，当校正了其它危险因素后尿酸对颈动脉斑块的影响程度明显减弱（P＞0．05）。结论血尿酸浓度升高虽可增加CAS的发病风险，但高尿酸血症并不是CAS斑块形成的独立危险因素。Objective To investigate the relationship between serum uric acid and carotid atheroscle-rosis （CAS）among the middle-aged and elderly people.Methods A total of 358 patients for physical exam-ination from January of 201 0 to December of 201 2.Patients were divided into two groups：CAS group （n=1 06）and normal group （n=252）.The 358 patients were further divided into 3 groups according to the se-rume uric acid （SUA）level,group A：SUA〈302 μmol/L （male）,〈268 μmol/L （female）;group B：SUA 302~354 μmol/L （male）,268~31 2 μmol/L （female）;group C：SUA 〉354 μmol/L （male）,〉31 2 μmol/L （female）.Intima-media thickness （IMT） T was detected via colour Doppler ultrasonograph. Results CAS Group with versus showed higher level of SUA compared with normal group （P〈0.05）.The SUA levels were related with the incidence of CAS （P〉0.05）,among which,the incidence of CAS in C group was 4 times higher than that of the normal group.However,logistic analysis showed lower influence of SUA on carotid plaque after adjusted with other influencing factors and showed no statistical significance （P＆gt;0.05）.Conclusion High uric acidemia can increase the incidence rate of CAS,but hyperuricacidemia was not independently correlated with CAS plaque.
