玉屏风散对迁延性腹泻患儿的免疫调节作用

摘　　要：目的:研究玉屏风散对小儿迁延性腹泻的治疗作用,探讨玉屏风散的免疫调节机制。方法:随机抽样将120例迁延性腹泻患儿分为研究组(玉屏风散+常规治疗)60例和对照组(常规治疗)60例,观察临床疗效,并采用ELISA法检测细胞因子水平,流式细胞术检测免疫球蛋白含量。结果:①研究组与对照组相比,在退热时间、呕吐停止时间上差异无统计学意义(P>0.05),但腹泻停止时间两组相比差异有统计学意义(P<0.05),研究组腹泻停止时间较对照组缩短;②研究组与对照组相比,细胞因子IFN-γ表达水平差异有统计学意义(P<0.05),研究组血清IFN-γ水平明显高于对照组;③研究组与对照组治愈后随访期体重增长、免疫球蛋白IgG水平差异无统计学意义(P>0.05),但腹泻复发率研究组明显低于对照组(P<0.05)。结论:玉屏风散可以提高迁延性腹泻患儿血清IFN-γ水平,发挥免疫调节功能,同时缩短患儿病程,降低腹泻病复发率。Objective： To study the role of Yupingfeng Powder in the treatment of children with persistent diarrhea, and investigate its immunomodulatory mechanism. Methods： 120 samples of children with persistent diarrhea were divided into two groups randomly ： study group （ Yupingfeng Powder ＋ conventional treatment） with 60 cases and control group （ conventional treatment ） with 60 cases, to observe the clinical efficacy and detect cytokine levels by ELISA method and flow cytometry. Results： ①Study group com-pared with the control group, difference of fever and vomiting stop time was not statistically significant （ P 〉 0.05 ）, but the diarrhea stop time of research group was shortened compared with the control group （P 〈 0.05） ; ②There was statistically significant difference on the serum IFN-γ level between study group and the control group （ P 〈 0.05）, the level of study group was significantly higher than that in the control group; ③There was no significant difference between study group and control group on the children body weight gain and IgG level （ P 〉 0. 05 ）, but diarrhea recurrence rate of the study group was significantly lower than the control group （ P 〈 0.05 ）. Conclusion： For the children with protracted diarrhea, Yupingfeng Powder can improve the serum IFN-γ level and play an immunomodulatory function, meanwhile, shorten the duration of disease and reduce diarrheal disease recurrence rate.
