超声引导下心包置管在急性心包填塞中的应用

摘　　要：目的探讨实时超声引导下心包置管引流在心脏介入术中并发急性心包填塞的应用价值。方法对20例在心脏介入术中并发的由超声心动图或及X线透视证实的急性心包填塞患者在实时超声引导下心尖部或心前区置入中心静脉导管抽吸或及引流。结果 20例急性心包填塞患者全部安全有效置管成功,患者经抽吸或引流后症状迅速缓解,得到有效救治。操作简单、快捷、并发症少。结论急性心包填塞是心脏介入诊疗过程中严重的并发症,及时和有效处理非常重要,在实时超声引导下经皮穿刺心包置管引流是治疗急性心包填塞的有效方法,而且方法简单、安全、有效,便于临床应用。Objective To investigate the application value of real-time ultrasound-guided percutaneous catheter drainage in acute cardiac tamponade during interventional cardiac surgery. Methods Twenty patients of concurrent cardiac interventional procedures by echocardiography or X-ray confirmed as acute cardiac tamponade were treated with real-time ultrasound-guided apical or precordial inserted central venous catheters drainage or aspi- ration. Results All the 20 patients had the catheter safely and successfully inserted. The symptoms were relieved af- ter aspiration or drainage The operation was simple, rapid, with few complications. Conclusion Acute cardiac tam- ponade is a serious complication ofinterventional cardiac surgery. Timely and effective treatment of acute cardiac tam- ponade is very important. Real-time ultrasound-guided percutaneous catheter drainage ofpericardial tamponade is an effective treatment for acute cardiac tamponade, which is simple, safe and effective for clinical application.
