摘  要:目的探讨视听统合训练对额叶颅脑损伤患者工作记忆和认知功能的影响。方法选取2012年在唐山市河北联合大学附属医院神经外科住院治疗后、经简易智能状态检查表(MMSE)筛查具有认知障碍的额叶颅脑损伤患者40例为研究对象,采用随机数字表法将其分为对照组和视听统合训练组各20例。对照组给予常规的认知康复指导;视听统合训练组给予视听统合训练,每天30—40min,5次/周。治疗3个月后,采用修订的韦氏记忆量表(WMS—RC)进行工作记忆评定,采用洛文斯顿作业疗法认知评定测验(LOTCA)进行认知功能的评价。结果视听统合训练组患者的故事理解、背数(顺背、倒背和数字广度)及积累分测验的评分均较对照组提高,差异均有统计学意义(P〈0.05)。视听统合训练组患者的定向、知觉、视运动组织、思维运作评分及LOTCA总评分均较对照组提高,差异均有统计学意义(P〈0.05)。结论视听统合训练可改善额叶颅脑损伤患者的工作记忆和认知功能。Objective To study the effects of visual and aural combined training on the working memory and cognition function of patients with frontal craniocerebral injury. Methods Forty inpatients diagnosed as having frontal craniocerebral injury and confirmed by mini - mental state examination (MMSE) as suffering from cognition disorder, were in 2012 divided according to random number table as trial group and control group, with 20 in each group. The patients in the control group were given guide of conventional cognition rehabilitation, while those in the trial group were given visual and aural combined training for 30 - 40 min daily, 5 times per week. The course was 3 months. Then working memory was evaluated using Wechsler Memory Scale ( WMS - RC) , and cognition function was evaluated using Loewenstein Occupational Therapy Cognitive Assessment (LOTCA) . Results Compared with the control group, the scores of comprehension, recited numbers and accumulated point test, as well as the total score in WMS - RC were higher (P 〈 0. 05) . And the scores of orientation, space, visual movement and thinking in LOTCA were higher (P 〈 0. 05) . Conclusion Visual and aural combined training can improve the working memory and cognition function in patients with frontal craniocerebral injury.
