国产紫杉醇联合体外高频热疗治疗恶性腹腔积液的效果观察

摘　　要：目的 观察国产紫杉醇腹腔内灌注化疗联合体外高频热疗治疗晚期恶性腹腔积液的效果.方法 收集山东省菏泽市牡丹人民医院肿瘤科2010年4月至2013年5月收治的恶性腹腔积液患者37例,根据治疗方法的不同分为观察组（18例）和对照组（19例）.观察组应用紫杉醇60 mg/m2＋温0.9％氯化钠溶液1 500 ～2 000 ml腹腔内灌注,1次/周;体外高频热疗（41 ～43℃）3次/周,lh/次.对照组应用顺铂40 mg/m2＋温0.9％氯化钠溶液1 500～2 000 ml腹腔内灌注,同观察组热疗.2组均每2周为1个周期,每2个周期评价疗效1次,总共治疗4周.结果 所有病例随访6个月,总有效率为51.4％（19/37）.观察组总有效率为66.7％ （12/18）,明显高于对照组[36.8％ （7/19）].观察组卡氏行为状态评分（KPS）治疗前为（60 ±5）分,治疗后为（80 ±5）分;对照组KPS评分治疗前为（60 ±5）分,治疗后为（70 ±5）分.2组患者治疗后KPS评分均高于治疗前（P＜0.05）,但组间差异无统计学意义（P＞0.05）.观察组不良反应发生率低于对照组[66.7％（12/18）比94.7％ （18/19）] （P ＜0.05）.结论 国产紫杉醇腹腔内灌注化疗联合体外高频热疗治疗恶性腹腔积液相对安全有效,可改善患者的生活质量.Objective To explore the peritoneal infusion chemotherapy of domestic paclitaxel complex external high frequency thermotherapy treating advanced malignant ascites.Methods All 37 malignant abdominal product liquid patients from April 2010 to May 2013 were collected and were divided into observation group（18 cases） and control group（19 cases）.The patients in observation group were treated with intraperitoneal perfusion （yew alcohol 60 mg/m2 ＋0.9% temperature chloride sodium solution 1 500-2 000 ml）,1 times/week,hot pedicures 3 times/week,41-43 ℃/times,1 h/time.The patients in control group were treated with intraperitoneal perfusion （cisplatin 40 mg/m2 ＋ 0.9% temperature chloride sodium solution 1 500-2 000 ml）,heat therapy 3 times/week,41-43 ℃/time,1 h/time,The cycle period was two weeks,and the effect was evaluated every two weeks.The course of treatment was 4 weeks.Results The total effective rate in observation group was higher than that in control group [66.7% （12/18） vs 36.8% （7/19）,P 〈 0.05].The scores of KPS in observation before treatment and after treatment were （60 ± 5）,（80 ± 5） scores,while in the control group they were （60 ± 5）,（70 ± 5）scores,respectively; there were significant differences after treatment in two groups （P 〈 0.05）.The incidence of adverse reaction in observation group was less than that in control group[66.7% （12/18） vs 94.7%（18/19）] （P 〈 0.05）.Conclusion The peritoneal infusion chemotherapy of domestic paclitaxel complex external high frequency thermotherapy in the treatment of malignant ascites is relatively safe and effective ; it can significantly improve the quality of life.
