经阴道超声联合尿促黄体生成素试纸监测不孕症患者自然周期卵泡发育及子宫内膜厚度的意义

摘　　要：目的 探讨应用经阴道超声联合尿促黄体生成素（LH）试纸监测不孕症妇女卵泡发育及子宫内膜生长情况,指导不孕患者适时同床的可行性.方法 对100例不孕妇女采用阴道B型超声、尿LH试纸进行排卵监测并记录子宫内膜的厚度及形态.结果 ①75例（75％）出现尿LH峰优势,卵泡直径平均（18.1±1.6） mm,尿LH峰后的48 h内B型超声显示排卵率为72.0％（72例）,B型超声监测和尿LH试纸显示排卵的时间比较一致.②根据卵泡发育情况超声图像表现排卵正常型（66例）、小卵泡型（9例）、卵泡黄体化不破裂型（16例）、卵泡生长过度形成囊肿型（3例）及无卵泡发育型（6例）.各种类型内膜生长均有其特征性表现.最终排卵75例中,24例怀孕,受孕率24.0％.24例排卵成功与51例排卵成功未妊娠患者妊娠卵泡直径和子宫内膜厚度差异有统计学意义[（21.8±1.4） mm比（20.1 ±1.7）mm,（12.4±1.6）mm比（9.1±2.4）mm,P＜0.01].结论 阴道B型超声监测和尿LH试纸联合检查在监测排卵、子宫内膜厚度及预测排卵时间等方面有重要的临床意义,方法简便,可行.Objective To detect the follicular development of infertilitas feminis and the thickness of internal membrane during natural period via the transvaginal ultrasound and luteinizing hormonetest paper.Methods According to the ultrasonography image characteristics,the maturity of the follicular and ovulation was recorded.The relation between endometrium and follicle growth was evaluated.Results ①The average diameter of dominant follicle was （18.1 ± 1.6） mm.After 48 hours of the summit of urine LH＆#39;,the rate ovulation was 86.9%.The transvaginal ultrasound and urine LH test paper were all predicting the ovulation time.②According to the follicular development,the sonogram had ovulation typiform,small ovarian follicle,ovarian follicle luteinization and ovarian follicle had the mark manifestation.Seventy-five cases （75.0%） had the urine LH peak and 24 cases had pregnancy; the pregnancy rate was 24.0%.Follicle diameter and endometrial thickness were shown in 24 cases of successful ovulation and 51 patients with successfulovulation [（21.8 ± 1.4） mm vs （20.1 ± 1.7） mm,（12.4 ± 1.6） mm vs （9.1 ±2.4） mm,P 〈0.01].Conclusions The transvaginal ultrasound and urine LH test paper can monitor infertilitas feminis follicular development and endometrium thickness.It can predict the ovulation time.
