关节镜下修整成形术治疗32例外侧盘状半月板的短期疗效分析

摘　　要：目的：观察关节镜下应用修整成形技术治疗32例外侧盘状半月板术后膝关节功能恢复情况并评价其临床疗效.方法：对我院在2008年7月～2012年12月应用关节镜修整成形术治疗的32例外侧盘状半月板患者的临床资料进行回顾性研究.手术时患者年龄为12～55岁,平均（29.3±177;16.1）岁；男18例、女14例；右膝17例、左膝15例.记录所有患者术前、术后的症状、体征和关节活动度,并采用Lysholm膝关节评分和国际膝关节文献委员会（IKDC）评分评价其膝关节功能.结果：32例患者术后获平均（15.75±177;5.50）个月随访,所有病例术后随访时症状均明显改善或消失,1例出现顽固性失眠,余无术后并发症.关节镜下半月板修整成形术后6个月Lysholm评分（96.28±177;3.28）和IKDC评分（91.78±177;4.24）值最高,与术前Lysholm评分（49.72±177;6.12）（t=-37.44,P＜0.01）和IKDC评分（47.64±177;4.04）（t=-51.88,P＜ 0.01）相比均有显著性提高.术后1个月时,18岁以下患者膝关节功能Lysholm评分（81.69±177;3.03）（t=4.79,P＜0.01）和IKDC评分（77.07±177;2.51）（t=-3.17,P＜0.01）显著优于18岁以上患者.结论：严格掌握手术适应症,应用关节镜下修整成形技术治疗外侧盘状半月板是良好恢复膝关节功能的可行方法.Objective To analyze the clinical outcome of arthroscopic plasty for discoid lateral meniscus. Methods From January 2008 to January 2012,32 patients with discoid lateral meniscus, including 18 males and 14 females,aged 29.3±16.1 years （12-55 years）were treated by arthroscopie meniscus plasty in our hospital. Their Lysholm and IKDC scores were evaluated before and after surgery. Results The patients were followed up for an average of 15.75±5.50 months （6 to 24 months）. The mean Lysholm score （96.28±3.28）and IKDC score （91.78±4.24）in 32 patients at the 6th month after surgery were significantly higher than that before surgery（49.72±6.12 and 47.64±4.04,respectively）（t=-37.44,P 〈 0.01 and t=-51.88,P 〈 0.01,respectively）. The Lysholm and IKDC scores of the patients aged less than 18 years were significantly higher than the patients aged over 18 years at the 1st month after surgery. Conclusions Arthroscopic meniscus plasty is a feasible technique for restoring knee function of patient with discoid lateral meniscus.
