摘　　要：目的研究应用锁定钢板加植骨内固定术治疗老年肱骨近端骨折的疗效。方法选取2007年12月～2011年2月南京军区福州总医院骨科收治的老年肱骨近端粉碎性骨折患者38例，按照Neer分型，2部分骨折18例，3部分骨折15例，4部分骨折5例。均行切开复位锁定钢板内固定并植骨，术后吊带悬吊，1周后开始功能锻炼。结果平均随访14．8个月，术后12个月38例患者肩关节活动情况，前举（148．4±12．3）。，外旋（39．1±14．9）。，内旋平均达第9胸椎水平。按Neer功能评分标准，优33例，良5例。结论对于老年骨质疏松，锁定钢板可提供牢固内固定，配合植骨可促进骨折愈合，早期功能锻炼对保护肩关节功能具有重要意义。Objective To explore the Clinical efficacy of treating the proximal humerus fractures of elderly patients with the locking plate and bone grafting. Methods 38 elderly patients with proximal humerus comminuted fractures were treated with open reduction and locked plate fixation from December 2007 to February 2011. According to Neer classification, 18 cases were two-part fractures, 15 cases were three-part fractures and 5 cases were four-part fractures. Sling suspensions were employed during recovery, and all patients began to take functional exercise one week after the operation. Results Thirty-eight patients were followed up for an average of 14.8 months. Statistics regarding shoulder joint activities after 12 months were as follows：arms forward （148.4 ±12.3）°, externa] rotation （39.1 ± 14.9）° and average pronation T9 level. According to the Neer function score, 33 cases were excellent and 5 cases were fine. Conclusion Locking plate can provide solid internal fixation for elderly osteoporosis,which promotes fracture healing when combined with the bone graft. Early functional exercise is of great significance to the protection of the functions of the shoulder.
