乳腺脓肿在彩色B超引导下小切口置管冲洗持续负压引流治疗的研究

摘　　要：目的：探讨乳腺脓肿在彩色B超引导下小切口置管冲洗持续负压引流治疗的临床效果。方法选取我院2012~2013年需手术治疗的54例乳腺脓肿患者作为研究对象，按照不同的治疗方法分治疗组和对照组，每组各27例，治疗组行彩色B超引导下小切口置管冲洗持续负压引流治疗，对照组行单纯切开引流术，比较两组患者愈合后切口长度、换药次数、愈合时间、疼痛程度、是否出现乳瘘以及是否继续母乳喂养等情况。结果治疗组患者切口长度、换药次数、愈合时间及疼痛度明显低于对照组，差异有统计学意义（ P＜0.05）。治疗组继续哺乳率为63.0%明显高于对照组的14.8%，差异有统计学意义（P＜0.05）。治疗组无患者出现乳瘘。结论乳腺脓肿在彩色B超引导下小切口置管冲洗持续负压引流治疗效果明显，相比于传统切开引流具有微创、简便、安全疗效等优点，值得推广应用。Objective To investigate the clinical effect of breast abscess by the small incision catheter for flushing and continuous negative pressure drainage treatment guided by color B ultrasound. Methods 54 cases of breast abscess needing operation treatment in our hospital during 2012-2013 were selected as the research objects, who were divided into the treatment group and the control group according to the different treatment methods, each group had 27 cases. The patients of the treatment group were given the small incision catheter for flushing and continuous negative pressure drainage treatment guided by color B ultrasound, while the patients of the control group were given only incision and drainage. The incision length after healing, dressing changing frequency, healing time, pain degree, whether breast fistula and whether continuing breastfeeding of patients in the two groups were compared. Results The incision length, dressing changing frequency, healing time and pain degree of patients in the treatment group were significantly lower than those in the control group, the differences were statistically significant（P＆lt;0.05）. The continuing breastfeeding rate of the treatment group was 63%, which was significantly higher than 14.8% of the control group, the difference was statistically significant（P＆lt;0.05）. No patients occurred breast fistula in the treatment group. Conclusion Breast abscess by the small incision catheter for flushing and continuous negative pressure drainage treatment guided by color B ultrasound has obvious treatment effect, and compared to the traditional incision and drainage, it has the advantages with minimally invasive, simple, safe and effective, and it is worthy of popularization and application.
