摘　　要：目的:评价EnSite 3000指导下7字线射频消融治疗房颤的临床疗效和安全性。方法:68例房颤患者(阵发性房颤46例,持续性房颤22例)在EnSite三维标测系统和肺静脉造影联合指导下重建肺静脉和左心房模型,后采用7字线消融术式予以射频治疗,观察并记录围术期和出院后患者房颤治疗效果和并发症发生情况。结果:术后随访12月,59例消融成功,总消融成功率86.8%,术中出现急性心包填塞1例,术后血管迷走神经反射2例,穿刺点血肿1例,经有效抢救后均脱离危险。结论:EnSite三维标测系统引导下的房颤7字线射频消融术具有较高的成功率和安全性,应用前景广阔。Objective：To evaluate the effectiveness and safety of radiofrequency ablation targeted at atrial fibrillation （AF） in stepwise linear approach guided by EnSite 3000. Methods： Sixty--eight AF patients （46 cases of paroxysmal AF and 22 cases of persistent AF） were reviewed. After the virtual reconstruction of pulmonary veins and left atrium under the guidance of EnSite 3000 and pulmonary phlebography, all pa- tients received radiofrequency ablation in stepwise linear approach. The efficacy and complications were ob- served and recorded after ablation. Results： After follow--up of 12 months, 59 out of 68 （86.8%） patients were succeeded in keeping sinus rhythm after radiofrequency ablation therapy. During ablation, the peri- cardial tamponade occurred in one patient. After procedure, the vasovagal reaction was found in two pa- tients and the hematomas at puncture site in one patient. Conclusion. The EnSite--navigated radiofrequen- cy ablation of atrial fibrillation in stepwise linear approach is safe and effective for the treatment of AF.
