双源CT双能量碘图显示犬急性心肌梗死的实验研究

摘　　要：目的评价双源CT(dual source computed tomography,DSCT)双能量碘图对犬急性心肌梗死的显示能力。方法采用开胸结扎左前降支的方法制备犬急性心肌梗死模型14只。制模完成后1小时对模型行DSCT双能量心脏扫描。心肌标本采用氯化三苯四氮唑(triphenyltetrazolium chloride,TTC)染色。对照TTC染色结果分析DSCT心肌融合图像及碘图。结果成功制备动物模型10只。对照TTC染色,DSCT心肌融合图像测得正常心肌平均CT值为114.5HU,梗死心肌平均CT值为79.90HU,两者间的差异有统计学意义(Z=-2.800,P<0.01),但肉眼无法明确梗死范围。DSCT心肌碘图测得正常心肌平均碘含量为1.67mg/ml,梗死心肌平均碘含量为0.05mg/ml,两者间的差异有统计学意义(Z=-2.810,P<0.01),肉眼可清晰识别梗死区范围。结论 DSCT心肌碘图显示梗死心肌的能力优于融合图像,具有临床应用潜能。Objective To evaluate the ability of iodine images to display acute myocardial infarction in canines with dual source computed tomography（DSCT）.Methods Fourteen canine models of acute myocardial infarction were produced by ligaturing the left anterior descending artery.One hour after the completion of ligation,the hearts of models were scanned by DSCT.Myocardial specimens were stained by triphenyl tetrazolium chloride（TTC）.According to TTC staining,myocardial fusion images and iodine images of DSCT were analyzed.Results Ten canine models were produced successfully.According to TTC staining,the average CT value of normal myocardium measured in iodine images was 114.5 HU,while that of myocardial infarction was 79.90 HU,there was statistical difference between them（Z=-2.800,P〈0.01）.But the naked eye could not distinguish the range of myocardial infarction.The average iodine concentration of normal myocardium measured in iodine images was 1.67 mg/ml,while that of myocardial infarction was 0.05 mg/ml,there was statistical difference between them（Z=-2.810,P〈0.01）,and the naked eye could distinguish the range of myocardial infarction clearly.Conclusion The DSCT myocardial iodine images are better than fusion images in displaying acute myocardial infarction.This technique has clinical potential.
