摘　　要：目的观测肝移植患者外周血中调节性T细胞(Treg)上CTLA-4、HLA-DR分子的表达特征,探讨CTLA-4、HLADR表达与术后急性排斥反应的关系。方法入组38例行肝移植术患者,其中16例患者发生急性排斥反应(急排组),22例未发生急性排斥反应(稳定组)。应用流式细胞仪检测两组肝移植术后患者外周血中Treg细胞上CTLA-4、HLA-DR分子的表达,并分析其与肝功指标的相关性。结果急排组外周血中Treg细胞上CTLA-4及HLA-DR分子表达水平明显低于稳定组(P<0.05)。此外,Treg细胞上CTLA-4及HLA-DR分子表达水平与肝功指标ALT、AST呈明显负性相关。结论肝移植患者发生急性排斥反应时,外周血中Treg细胞上CTLA-4及HLA-DR分子表达水平降低,提示其可能与术后急性排斥反应的发生相关。Objective To investigate the characteristics of CTLA-4 and HLA-DR molecular expression on regulatory T cells （Treg） in the peripheral blood of liver allograft recipients, and to discuss the correlation between the expression and postoperative acute rejection. Methods Thirty eight liver allograft recipients were enrolled,in which 16 ones suffered acute rejection （AR group） and 22 ones had no such reaction （ stable group）. Flow cytometry was used to detect the expression of CTLA-4 and HLA-DR molecule on Treg in the peripheral blood of the patients in both groups. The correlation between the expression and the liver function indexes was analyzed. Results The expression levels of both CTLA-4 and HLA-DR molecule on Treg in AR group was significantly lower than those in the stable group （P 〈 0.05 ）. Furthermore, there was an obviously negative correlation between the expression levels of CTLA- 4 and HLA-DR molecule on Treg and the levels of ALT and AST. Conclusion When acute rejection occurs in the liver allograft recipients, the expression levels of CTLA-4 and HLA-DR molecule on regnlatory T cells in their peripheral blood will decrease, which may be induced by the postoperative acute rejection.
