低温等离子刀扁桃体减容联合腺样体消融术治疗儿童鼾症疗效观察

摘　　要：目的 评估低温等离子扁桃体减容联合腺样体消融术治疗儿童阻塞性睡眠呼吸暂停低通气综合征（OSAHS）的疗效.方法 2008年1月~2013年5月我科诊治126例3~12岁OSAHS患儿,按照手术方式不同分为治疗组（59例）与对照组（67例）,治疗组采用低温等离子刀扁桃体减容术＋腺样体消融术,对照组采用低温等离子刀扁桃体切除术＋腺样体消融术,以术中出血量、术后疼痛评分及术后6个月疗效为评价指标,比较两组疗效.结果 治疗组术中出血量、术后疼痛评分均低于对照组（P＜0.05或P＜0.01）,两组术后6个月疗效比较无统计学差异.结论 低温等离子刀扁桃体减容联合腺样体消融术具有术中出血少、术后疼痛轻的优点,治疗儿童OSAHS可获得较好的疗效.Objective To evaluate the clinical curative effect of hypothermia plasma tonsils volume reduction combined with adenoid ablation on children with obstructive sleep apnea hypopnea syndrome （OSAHS）. Methods 126 OSAHS children aged from 3 to 12 who were admitted to our department between January 2008 and May 2013 were divided into treatment （59 eases） and control groups （67 cases） according to the different surgical ways. The treatment group received hypothermia plasma tonsils volume reduction combined with adenoid ablation, and the control group received hypothermia plasma tonsillectomy combined with adenoid ablation. Comparison was made in the curative effects between the two groups based on the evaluation indexes of perioperative bleeding volume, postoperative pain score,and therapeutic effect of 6 months later. Results The perioperative bleeding volume and postoperative pain score in the treatment group were lower than those in the control group （P 〈 0.05 or P 〈 0.01 ）, but there was no significant difference in the therapeutic effect of 6 months later between the two groups. Conclusion Hypothermia plasma tonsils volume reduction combined with adenoid ablation has the advantages of less bleeding and mild postoperative pain and have good curative effect on children with OSAHS.
