输尿管镜结合EMS四代弹道联合超声碎石机治疗慢性精囊炎的疗效观察

摘　　要：目的：评价经输尿管镜结合EMS四代弹道联合超声碎石机治疗慢性精囊炎的临床效果。方法：选择慢性精囊炎患者15例，单侧14例，双侧1例。15例均伴有会阴部不适，14例伴有血精，9例伴有前列腺炎，6例合并精囊结石。术前均通过直肠超声和CT检查确诊。利用EMS四代弹道联合超声碎石机的吸附功能吸出不同黏稠度的感染精浆，遇有黏稠成块的精浆及结石时启动超声碎石装置，击碎结石并吸出。结果：手术时间20～60min，住院时间3～12d。术后随访4～12周。血精全部消失，14例会阴部不适缓解，前列腺炎4例治愈。1例术后3天出现双侧附睾睾丸炎，给予抗炎对症治疗后缓解。结论：输尿管镜结合EMS四代弹道联合超声碎石机治疗慢性精囊炎具有安全性高，微创，无精囊和直肠损伤等并发症，对感染的精浆及结石清除彻底，可最大限度地减少术后复发。在吸附感染精浆的同时可进行碎石取石，值得临床进一步推广。Objective：To appraise the clinical effects of transurethral seminal vesiculoscopy and 4th EMS in the treatment of chronic seminal vesiculitis. Method：Fifteen patients with chronic seminal vesiculitis included 14 cases with unilateral disease and one case with bilateral diseases. All cases suffered from perineal region complaint. Fourteen of them were found hematospermia. Nine of them were shown prostatitis. Six cases were discovered se- minal stones. Diagnoses were confirmed by rectum ultrasound and CT before operation. Fourth EMS was able to absorb the different viscidity infected seminal plasma. When the process of absorption appeared unsuccessful, ul- trasound lithotripsy procedure was applied. Result： Operation time was 20-60 rain and hospital stay was 3-12 d. Four to twelve weeks follow-up results showed： all the hematospermia and four prostatitis patients were cured; 14 cases with perineal region complaint released. A case was found testitis and epididymitis three days after the opera- tion and cured with anti-inflammatory treatment. Conclusion：The procedure of transurethral seminal vesiculoscopy and 4th EMS in the treatment of chronic seminal vesiculitis is worth promoting for its safety, minimal invasion, no seminal, rectal or other injuries. Moreover, infected seminal plasma and stones can be completely cleaned simultaneously.
