经直肠超声引导前列腺＂10＋X＂点穿刺活检术的临床价值

摘　　要：目的 探讨经直肠超声引导下前列腺＂10＋X＂点法穿刺活检术在诊断前列腺疾病中的临床价值.方法 对89例经前列腺特异抗原检测或直肠指诊或影像学检查异常的患者,行超声引导下经直肠10点前列腺穿刺活检加可疑区域1～2针活检.结果 经病理证实前列腺癌34例（占38.2%）,前列腺增生21例（占23.6%）,前列腺上皮内瘤19例（占21.3%）,前列腺炎15例（占16.9%）.89例接受穿刺的患者23例术后出现血尿,9例出现血便,1例出现发热,无1例出现严重的并发症.结论 经直肠超声引导下行前列腺＂10＋X＂点穿刺活检术,穿刺成功率高、术后并发症少,有助于前列腺疾病的定性诊断,具有很高的临床价值.Objective To explore the clinical value of trans - rectal ultrasound - guided prostate biopsy by ＂ 10 ＋ X＂ points method in the diagnosis of prostate disease. Methods Uhrasouml - guided trans - rectal prostate biopsies were performed on 89 patients who had abnormal level of prostate speeific antigen or who had prostate nodules by digital rectal examination or on imaging. Results Pathology proved prostate carcinoma in 34 cases （38.2%）, benign prostatic hyperplasia 21 cases （23.6%）, prostatic intraepithelial neoplasia 19 cases （21.3%）, prostatitis 15 cases （I6.9%）. A total of 89 cases underwent ＂ 10 ＋ X＂ cores prostate biopsy, there were 23 cases with hematuria, 9 cases with hema- tochezia, 1 case with fever after operation. No severe complications could be found among the patients underwent trans - rectal ultrasound guided ＂ l0 ＋ X＂ cores prostate biopsy. Conclusions Trans - rectal ultrasonic guidance in prostatic puncture biopsy by ＂10 ＋ X＂ points method has advantages of higher puncture achievement ratio and fewer postoperative complications. It is helpful for the qualitative diagnosis of prostatic disease and is highly valuable in clinical application.
