经阴道超声造影对子宫内膜息肉的研究

摘　　要：目的探讨子宫内膜息肉的超声造影特点、时间-强度曲线参数特征及其临床价值。方法对59例病理诊断为子宫内膜息肉患者的超声造影图像特征进行总结分析,并对感兴趣区域制作时间-强度曲线(TIC),比较病灶与正常内肌层在曲线参数上的差异。结果超声造影诊断内膜息肉的敏感度、特异性、阳性预测值、阴性预测值、准确率分别为86.4%、85.7%、81%、90%、86%。超声造影较传统超声诊断敏感度、阳性预测值、阴性预测值、准确率都有统计学意义。灌注时相上59例患者均表现为非早进早退型;灌注方式上93.2%(55/59)表现为离心型,增强强度与均度上表现多样;滋养动脉显示率为81.4%(48/59),时间-强度曲线分析结果显示,子宫内膜息肉与宫壁内肌层TIC曲线各参数差异无统计学意义。结论子宫内膜息肉的超声造影及TIC曲线特点,可为子宫内膜息肉的诊断和鉴别诊断提供有价值的信息。Objective To observe the enhancement pattern and time - intensity curve of endometrial polyps by using contrast - en- hanced transvaginal sonography（CEUS） and to assess the clinical value of it in endometrial polyps. Methods Fifty - nine cases of uterine endometrial polyps patients, which were confirmed by pathology, were enrolled in our study. To the imaging features of CEUS in endome- trial polyps were investigated, and the regions of interest to make TIC was choosed, then the endometrial lesion with normal myometrium was compared. Results The sensitivity, specificity, positive predictive value （PPV） , negative predictive value （NPV） and accuracy of CEUS for the diagnosis of endometrial polyps were 86.4% ,85.7% ,81% ,90% ,86% , respectively. There were significant differences in sensitivity, PPV, NPV and accuracy between CEUS and general ultrasound. After contrast administration, all the 59 cases didn＇t show rapidly washing in and out. 93.2% （55/59）cases showed the enhancement pattern was centrifugal enhancement. The enhancement inten- sity appeared variously. 81.4% （48/59） eases showed the recognizable feeding arteries. Time -intensity curve analysis showed that there was no significant difference in four parameters between endometrial polyps and the innermost myometrium. Conclusion There are some characteristic points in CEUS and TIC of endometrial polyps. They can provide us richer and more reliable information in diagnosis and dif- ferential diagnosis of endometrial polyps.
