TNF-α及IL-6对类风湿关节炎并发糖代谢异常相关研究

摘　　要：目的探讨类风湿关节炎(RA)并发糖代谢异常与细胞因子IL-6及TNF-α之间的相关性。方法选取类风湿关节炎并发糖代谢异常患者30例、单纯RA患者40例、T2DM患者30例及健康对照组30例,采用酶联免疫吸附法测定血清中IL-6及TNF-α水平,采用SPSS20.0软件进行统计学分析。结果 RA并发糖代谢异常组血清中IL-6、TNF-α水平明显高于单纯RA组(P<0.01、P<0.01),RA患者血清IL-6、TNF-α水平与DAS28疾病活动度评分呈正相关(r值分别为0.63、0.59,P均<0.01),RA患者的HOMA-IR值与DAS28疾病活动度评分呈正相关(r=0.55,P<0.01)。结论 RA患者血清中IL-6及TNF-α的水平明显升高、且与糖代谢异常的发生密切相关,并且IL-6及TNF-α与RA患者的疾病活动度呈正相关。Objective To explore the correlation of cytokines such as IL-6 and TNF-a in the rheumatoid althritis （RA） supervene with alnormal glucose metabolism. Methods Select 30 RA patients supervene with impaired glucose metabolism,40 merely RA patients without other diseases ,30 Type 2 Diabetes （T2DM）and 30 health control group. Each of them adopt ELISA to test serum level of IL-6 and TNF-a. Results The serum level of IL-6 and TNF-a in the RA supervene with Alnormal glucose metabolism group are significantly higer than merely RA group （P〈0.01 ,P〈0.01.respectively）, Meanwhile,in the RA patients, serum level of IL-6 and TNF-a were positively correlated with DAS28 （r=0.63,r=0.59,respectively,P〈0.01）, also, HOMA-IR was significantly positively correlated with DAS28 （ r=0.55 ,P〈0.01）. Conclusion The serum level of IL-6 and TNF-a in the RA patients are obviously and closely related to the happening of alnormal glucose metabolism, and were positively correlated with RA disease activity.
