加味桃红四物汤对腰椎术后血液高凝状态影响的临床研究

摘　　要：目的:观察加味桃红四物汤治疗腰椎术后血液高凝状态的临床疗效。方法:将83例患者按照随机数字表法分为对照组41例和治疗组42例。对照组术后给予七厘胶囊口服;治疗组术后给予加味桃红四物汤口服。观察术前、术后第1天、第5天、第10天抽血检查凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)及D-二聚体(D-D)的变化。结果:两组术后APTT均无明显变化;术后第5天PT均较术前缩短,组间差异无统计学意义(P>0.05);对照组术后10天PT进一步缩短,治疗组变化不明显,组间比较,差异有统计学意义(P<0.05);FIB和D-D与对照组比较,差异有统计学意义(P<0.05);术后10 d治疗组下肢深静脉血栓发生率明显低于对照组,差异有统计学意义(P<0.05)。结论:加味桃红四物汤可有效改善腰椎术后血液高凝状态,降低术后血浆D-D水平,未发现明显不良反应。Objective：To observe the Modified Taohongsiwutang clinical efficacy of lumbar hypercoagulable state after treatment. Methods ：83 patients randomly divided into 42 cases in the control group 41 cases and control groups. The control group was given oral qili capsules;treatment group were given flavored Taohongsiwutang orally. Observed before postoperative day 1, the first five days, the first 10 days blood tests prothrombin time （PT） , activated partial thromboplastin time （APTT）, fibrinogen （FIB） and D-dimer（ DD ） changes. Results：There were no significant changes in postoperative APTT;former PT than 5 days after surgery to shorten between the two groups was not statistically significant（ P 〉 0.05 ） difference ~ the control group after 10 days PT further shorten the treatment group changes ob- vious between the two groups, the difference was statistically significant（ P 〈 0.05 ） ;FIB and DD with the control group, the difference was statistically significant（ P 〈 0.05 ） ;after 10 d treatment group the incidence of deep venous thrombosis was significantly lower than the control group, the difference was statistically significant （ P 〈 0.05 ）. Conclusion ： Modified Taohongsiwutang lumbar spine surgery can be effective in improving blood hypercoagulable state, reduce postoperative plasma DD levels and found no significant adverse reactions.
