摘 要:目的了解铜绿假单胞菌在临床标本中分布及耐药性,为临床治疗提供依据。方法对医院2009-2012年临床各类标本分离的铜绿假单胞菌进行统计分析,并对铜绿假单胞菌采用珠海迪尔生物工程有限公司生产非发酵菌板条对菌株进行鉴定和药敏试验。结果 4年分离铜绿假单胞菌主要来源于痰液标本占86.2%;主要集中在ICU和呼吸内科,分别占42.8%和32.7%;铜绿假单胞菌对头孢哌酮/舒巴坦、左氧氟沙星、环丙沙星耐药率最低,分别为2.1%、5.2%、7.1%,其次为亚胺培南、美罗培南、哌拉西林/他唑巴坦、头孢吡肟、阿米卡星、妥布霉素,耐药率分别为10.0%、10.0%、10.2%、12.5%、14.2%、15.8%,对其他抗菌药物耐药率较高。结论铜绿假单胞菌对多种抗菌药物均产生不同程度的耐药性,临床应加强其耐药性监测及针对性管理措施,减少耐药性菌株产生及扩散。OBJECTIVE To understand the distribution of Pseudomonas aeruginosa in the clinical specimens and analyze the drug resistance so as to guide the clinical treatment. METHODS The P. aeruginosa strains isolated from various clinical specimens between 2009 and 2012 were statistically analyzed, then the strains were identified with the use of non-fermenting bacteria batten produced by Deere Biological Engineering Co. , Ltd. of Zhuhai , and the drug susceptibility testing was performed. RESULTS Of all the P. aeruginosa strains isolated during the 4 years, 86. 2 % were from the respiratory tract sputum specimens, among which 42. 8 % were from ICU, 32. 7 % from department of respiratory medicine. The drug resistance rates of the P. aeruginosa to cefoperazone-sulbactam, levofloxacin, and ciprofloxacin were 2. 1 %, 5. 2 %, and 7. 1 %, respectively; while the drug resistance rates to imipenern , meropenem, piperacillin-tazobactam , cefepime , amikacin , and tobramycin were 10. 0%, 10. 0%, 10.2%, 12.5%, 14.2%, and 15.8%, respectively; the drug resistance rates to other antibiotics were relatively high. CONCLUSION The P. aeruginosa strains vary in the drug resistance to many antibiotics; it is necessary for the hospital to strengthen the monitoring of drug resistance and take targeted intervention measures so as to prevent the emergence and spread of the drug resistant strains.
