质量管理在内镜清洗消毒流程中的运用

摘　　要：目的对内镜清洗消毒环节质量的关键问题进行分析和优化,提高内镜清洗消毒合格率和医务人员依从性,保证内镜清洗消毒质量和安全。方法收集消化内镜中心2010年1月-2011年12月再处理内镜记录共104 740例次,分析并找出影响清洗消毒的主要问题和关键因素,通过流程优化、细化管理和过程控制等措施,对清洗消毒流程进行改进,并收集2012年1-12月的内镜清洗消毒共62 147次相关资料进行比较,评价其效果。结果通过12个月的流程改进和过程控制,内镜清洗消毒5大步骤未有新的问题出现,内镜钳子管维修、内镜表面黄斑、纱布损耗等问题均不同程度减少,清洗刷和清洗剂更换频率增高,职业防护检查和细菌学监测合格率均达到100.0%。结论控制内镜清洗消毒关键环节质量,不仅提高了清洗消毒流程的依从性,还保证了内镜清洗消毒的质量,为患者诊疗工作的快速、安全提供了保障。OBJECTIVE To analyze the key problems in the cleaning and disinfection of endoscopes and optimize the quality management and improve qualified rate of cleaning and disinfection of endoscopes and compliance of medical staff so as to ensure the quality and safety of the cleaning and disinfection. METHODS From Jan 2010 to Dec 2011, the records of 104 740 case times of cleaning and disinfection of endoscopes were collected from the digestive endoscopy center, then the major problems in the cleaning and disinfection were found out, the disinfection and cleaning process was improved via the optimization of process, detailed management, and process control , the relevant data of 62 147 case times of cleaning and disinfection were collected from Jan to Dec 2012, and the effect was evaluated. RESULTS Through a 12-month process rectification and process control, the new problems have not emerged in the five steps of cleaning and disinfection of endoscopes; the maintenance of endoscopic forceps tube, yellow spots on surfaces of endoscopes, and loss of gauze have reduced in different degrees, the frequency of replacement of cleaning brush and cleaning agents increased, and the qualified rates of both the occupational protection examination and the bacteriological surveillance reached up to 100. 0 %. CONCLUSION The control of key links of the cleaning and disinfection of endoscopes can not only improve the compliance of the cleaning and disinfection process but also ensure the quality of the cleaning and disinfection, which guarantees the rapid and safe diagnosis and treatment.
