参附汤治疗慢性心力衰竭临床疗效评价

摘　　要：目的 探讨参附汤治疗慢性心力衰竭患者的临床疗效和安全性.方法 选择2010年1月～2011年10月在贵州省惠水县人民医院内科病房住院的116例慢性心力衰竭患者,随机分为治疗组60例,采用最佳西医方案＋口服参附汤治疗;对照组56例,采用最佳西医方案＋口服安慰剂汤治疗.两组服药方法为每日3次,每次50 ml,每日总剂量150 ml,疗程为6个月.评估慢性心力衰竭患者再次住院情况、中医症状改善情况及不良反应.结果 随访时间为1年,两组病例均全部随访.治疗组再次住院情况与对照组比较,差异有统计学意义（P＜0.01）;治疗6个月后,治疗组心悸、气短、面肢浮肿、口干、气喘、胸闷、咳嗽等症状改善情况与对照组比较,差异有统计学意义（P＜0.01）;随访3个月、6个月、9个月、12个月,治疗组在心悸、气短、面肢浮肿、口干、气喘、胸闷、咳嗽等症状改善情况分别与同时段对照组比较,差异均有统计学意义（P＜0.01）.结论 参附汤治疗慢性心力衰竭均能改善患者的心功能和临床症状,减少再次住院次数,且不良反应少.Objective To explore the clinical effect and safety of Shenfu decoction on patients with chronic heart failure.Methods 116 cases with chronic heart failure, who were admitted to internal medicine wards of the People＇s Hospital of Huishui County from Jan,2010 to Oct, 2011, were divided into the treatment group（60 cases） and the control group（56 cases）.The former were given the best Western medicine treatment and oral ad- ministration of Shenfu decoction, while the latter were given the best Western medicine treatment and oral administration of Placebo decoction. All patients took medicine 3 times a day with 50 ml each time for 6 months.Then, the rehospitalization,improvement of TCM symptoms and adverse reactions of both groups were evaluated.Results Both groups were followed up for one year, which showed that the difference of rehospitalization between groups was statistically significant（ P 〈0.01）. After 6 months of treatment,the treatment group had significantly greater improvement in symptoms like palpitations, shortness of breath, edema in faces and limbs, dry mouth, asthma,chest tightness and cough than the control group （ P 〈 0.01 ）. Compared with the control group, the treatment group also had more significant improvement of above symptoms in the 3rd month, 6th month, 9th month, and 12th month of follow-up visit after treatment were respectively compared with those in the same periods of the control group（ P 〈0.01）.Conclusion Shenfu decoction can not only improve cardiac function and clin- ical symptoms of patients with chronic heart failure,but also reduce the times of rehospitalization and adverse reaction.
