摘  要:目的观察米索前列醇联合缩宫素预防产后出血的效果。方法选择住院分娩的260例产妇,随机分为观察组和对照组各130例,观察组胎儿娩出后立即口服米索前列醇600μg,同时静滴缩宫素20u;对照组胎儿娩出后立即静滴缩宫素20u,观察比较两组产后2h和24h出血量及用药不良反应。结果观察组产后2h出血量(207±53)ml,产后24h出血总量(320±65)ml,对照组产后2h出血量(351±79)ml,产后24h出血总量(425±89)ml,观察组均明显少于对照组(P<0.01)。两组均有轻度胃肠道反应,但比较差异无统计学意义(P>0.05)。结论米索前列醇联合缩宫素较单纯使用缩宫素能有效促进子宫收缩,可有效减少产后出血量,不增加不良反应。Objective To observe the curative effect of misoprostol combined with oxytocin in preventing postpartum hemorrhage.Methods 260 cases of pregnant women, who chose to deliver in our hospital,were randomly divided into the observation group and the control group with 130 cases in each group. The observation group were given 600 μg of misoprostol( by oral administration) and 20 u of oxytocin (by intravenous injection) immediately the babies were delivered.The control group were given 20 u of oxytocin (by intravenous injection) immediately the babies were delivered.Then,the amount of bleeding 2 h and 24 h after delivering and adverse drug reactions of both groups were observed.Results The amount of bleeding 2 hours after delivering in the observation group was (207±53)ml, and the amount of bleeding 24 h after delivering was (320--65)ml.But those of the control group were (351±79)ml and (425 ± 89)ml, respectively, which were significantly more than those of the control group( P 〈0.01). Both groups had mild gastrointestinal reactions, but no significant statistical differences were found( P 〉0.05).Conclusion Misoprostol combined with oxytocin can promote uterine contraction more effectively than oxytocin. They can effectively reduce the amount of postpartum hemorrhage without increasing adverse events.
