自制复方黄连液冰敷动静脉穿刺所致皮下血肿的效果研究  

Effect of Self-made Compound Coptis Liquid on Icing Treatment of Subcutaneous Hematoma due to Arteriovenous Puncture

摘　　要：目的观察并探讨自制复方黄连液冰敷治疗动静脉穿刺处的皮下血肿的临床效果。方法入选2010年6月—2011年6月老年综合内科住院患者发生动、静脉穿刺处皮下血肿的90例患者为研究对象,随机分为观察组、对照1组、对照2组,每组各30例,3组患者血肿处分别应用自制复方黄连液持续冰敷、50%硫酸镁冷热湿敷、生马铃薯片冷敷,治疗24 h时观察3组患者血肿肿胀程度、疼痛指数与治疗效果。结果 3组患者总体疗效比较差异有统计学意义(P<0.05);3组患者治疗后总体疼痛程度比较差异有统计学意义(P<0.05);观察组疼痛程度轻于对照1、2组(P<0.05);治疗后3组肿胀程度均较治疗前显著降低(P<0.05),治疗后3组肿胀程度比较差异有统计学意义(P<0.05),观察组与对照1组和对照2组比较差异均有统计学意义(P<0.05),对照组1和对照组2比较差异无统计学差异(P>0.05)。结论应用自制复方黄连液持续冰敷治疗动静脉穿刺部位的血肿,可显著缓解肿胀和疼痛。Objective To observe and discuss the clinical efficacy of self-made compound Coptis liquid in icing treatment of subcutaneous hematoma resulting from arteriovenous puncture. Methods Ninety admitted elder patients with subcutaneous hematoma due to arteriovenous puncture from June 2010 to June 2011 were randomized into observation group, control group 1 and control group 2, with 30 cases in each group. Patients in three groups received sustaining icing therapy with self-made compound Coptis liquid, cold and wet compressing therapy by 50% magnesium sulfate and cold compress by raw potato slices respectively. Hematoma swelling, pain index and clinical efficacy of three groups were recorded and compared after 24h treatment. Results Effective differences among observation group and control groups were statistically significant （ P〈0.05）; Treatment efficiency on pain alleviation in observation group was significantly higher than that in control groups （ P〈0.05）; Swelling decreased significantly after 24h treatment （P〈0.05） and the effect was more obvious in observation group than that in control group1 and control group 2, and the differences among groups were statistically significant. Conclusion Self-made compound Coptis liquid can significantly relieve swelling and pain for arteriovenous hematoma due to puncture, which is worthy of promotion for its significant efficacy.
