摘 要:目的观察不同剂量高压氧预处理对大鼠肝脏缺血再灌注损伤的作用,并探讨其机制。方法 40只SD大鼠随机分成4组,对照组(C组)、缺血再灌注组(I/R组)、高压氧预处理1次组(D1组)、高压氧预处理2次组(D2组)。分别对D1和D2组大鼠进行高压氧预处理,连续10日后建立肝脏缺血再灌注损伤模型,检测各组血清中丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、超氧化物歧化酶(SOD)以及组织中Na+-K+ATP酶的活力。光学显微镜下观察各组大鼠肝脏组织结构的变化。结果与对照组相比,I/R组大鼠肝功能损害明显;D1组大鼠ALT和AST活力较I/R组显著降低,SOD和组织中Na+-K+ATP酶活力显著升高;D2组大鼠ALT和AST活力较I/R组显著升高,SOD和组织中Na+-K+ATP酶活力显著降低。光学显微镜下D1组肝细胞轻度水肿,嗜酸性变减轻,肝窦内轻度瘀血;D2组肝脏结构紊乱,肝细胞片状水肿,可见大量空泡状,灶状嗜酸性变,肝窦瘀血明显。结论适度剂量的高压氧预处理可减轻大鼠缺血再灌注肝脏损伤,而过高剂量的高压氧预处理可加重肝脏损伤。[ Objective ] To study the effects of different doses of hyperbaric oxygen preconditioning on hepatic reperfusion injury in rats, and investigate the mechanism. [ Methods] Forty SD rats were divided into 4 groups randomly, control group (C group), is- chemia reperfusion group (I/R group}, HBO preconditioning 1 time group (D1 group), HBO preconditioning 2 times group (D2 group). The vascular pedicle of the liver was obstructed to establish liver I/R model after the rats pretreated with hyperbaric oxygen in D1 group and D2 group for 10 days. Plasma alanine aminotransferase (ALT) , aspartate aminotransferase (AST) , superoxide dismutase (SOD) and liver tissue Na + -K + ATPase of each group were measured. The structure of liver was observed by light mi- croscopy. [ Results] Compared with the control group, the hepatic injury in rats were obvious in I/R group; ALT and AST level in group D1 were decreased significantly than group I/R, SOD level and Na + -K+ ATPase in liver organization in group D1 were in creased significantly than group I/R; ALT and AST in group D2 were increased significantly than group I/R,SOD and Na + -K + ATPase in group D2 were decreased significantly than group I/R. Under microscope, mild edema to hepatocytes, eosinophilic variant miti- gation, mild bleeding in the hepatic sinusoid were observed in group D1 rats; liver structural disorder, hepatocellular flake edema, spreading vacuoles in the cytosol and extracellular space, focal eosinophilic variant and blood congestion in the hepatic sinusoid were observed in D2 group. [ Conclusion] Moderate doses of the hyperbaric oxygen can protect the rats liver against ischemia-reper- fusion injury, but higher doses of the hyperbaric oxygen may aggravate hepatic injury.
