摘 要:目的 观察初诊2型糖尿病病人体内白细胞介素-1B(I L-1B)含量变化,分析其变化与胰岛B细胞早相分泌功能的关系。方法 2010年1月)2011年6月,选取青岛大学附属医院内分泌门诊初诊2型糖尿病病人56例作为糖尿病组,另选择同期门诊健康体检者38例作为正常对照组。测定I L-1B、空腹血糖、血脂并同期进行精氨酸试验。两因素间关系采用Pearson相关分析,多个变量间的分析采用多元逐步回归分析。结果 两组间I L-1B、稳态模型胰岛素抵抗指数(HO MA-I R)、胰岛素第一时相峰值倍数、稳态模型胰岛B细胞功能指数(HO MA-B)差异有统计学意义(t=4.063~6 9.78,P〈0.05),体质量指数、年龄差异无统计学意义(P〉0.05)。两组IL-1B与空腹血糖、HO MA-I R均呈正相关(r=0.327、0.204,P〈0.05),与胰岛素第一时相峰值倍数、HO MA-B均呈负相关(r=-0.243、-0.392,P〈0 0.5)。校正性别、年龄、家族史、血脂、血压后,I L-1B、Hb A1c、体质量指数是HO MA-B的影响因素(F=26.435,P〈0.01)。结论 初诊2型糖尿病病人IL-1B水平明显升高,与胰岛B细胞早相分泌功能呈负相关。体内I L-1B水平升高可能影响胰岛B细胞早相分泌功能。Objective To observe changes of interleukin-1β (IL-1β) in newly diagnosed type 2 diabetes (T2D), and analyze its relationship with β-cell first-phase secretion function. Methods From January 2010 to June 2011, 56 patients with new-ly-diagnosed T2D (T2D group) were selected from outpatients who visited the Affiliated Hospital of Qingdao University Medical College, and 38 healthy individuals who came for a health examination at the corresponding time period served as controls. IL-1β, fasting blood glucose (FBG), and blood lipids were detected, and L-arginine stimulation test was concurrently conducted. The Pearson correlation coefficients were calculated to assess the strength of the correlations of random pairs of parameters. Multiple stepwise regression analysis was performed to determine association between multiple factors. Results There were significant differences between the two groups in terms of IL-1β, homeostasis model assessment of insulin resistance (HOMA-IR), the insulin secretion peak and HOMA-β (t =4.063-6.978,P〈0.05), but no statistical differences were noted between them with regard to BMI and age (P〈0.05). The IL-10 in both groups was positively correlated with FBG and HOMA-IR (r=0.327,0.204;P〈0.05), and negatively correlated with the insulin secretion peak and H OMA-β (r =- 0.243,-0.393;P〈0.05). Multiple stepwise regression analysis indicated that H OMA-β was affected by IL-1β, H bAlc and BMI (F = 26.435, P〈0.01) after adjustment of age, sex, family history of diabetes, blood lipids, and blood pressure. Conclusion The levels of IL-10 in newly diagnosed T2D patients are markedly elevated and negatively correlated with β-cell first-phase secretion function. IL-β level in vivo is probably to affect β-cell first-phase secretion function.
