摘  要:目的 探讨阿托伐他汀对肾间质纤维化的作用及其机制。方法 以单侧输尿管梗阻(U U O)的Wist ar大鼠为肾间质纤维化模型,将54只大鼠分为假手术组(S O R)、U U O组及U U O+阿托伐他汀组(治疗组),分别给予相应处理。于术后第1、2、3周观察3组大鼠肾小管损伤程度和肾间质纤维化指数;术后第2、3周采用免疫组化染色法,检测大鼠肾脏组织转化生长因子-B(T GF-B)、S mad2蛋白、纤维连接蛋白(F N)及结缔组织生长因子(CT GF)的表达水平。结果 第1、2、3周U U O组、治疗组肾小管损伤程度及肾间质纤维化指数均大于S O R组,治疗组肾小管损伤程度及肾间质纤维化指数均小于UUO组,差异有显著性(F=9 0.09~11.231,P〈0.05)。第2、3周3组间TGF-B1、S mad2、F N及CT GF水平比较,U U O组最高,S OR组最低,差异有显著性(F=10.365~11.775,P〈0.05)。结论 阿托伐他汀能显著减轻U U O导致的肾间质损伤和纤维化倾向,其作用机制与抑制T GF-B1等表达有关。Objective To investigate the protective effect of atorvastatin on renal interstitial fibrosis (RIF) and its mech- anism. Methods A rat model of RIF was created in 54 Wistar rats using unilateral urethral ligation, which were equally ran- domized to three groups as sham operation group, model group and treatment group, and treated accordingly. The degree of injury of renal tubule and the index of RIF were observed at weeks 1, 2 and 3 after operation; the expressions of TGF-β1, Smad2, FN and CTGF were detected (using immunohistochemical staining) at weeks 2 and 3 after operation. Results At weeks 1, 2 and 3, the degree of injury of renal tubule was more severe and the index of RIF was higher in model group and treatment group than the sham operation group, the degree of injury of renal tubule was milder and the index of RIF was lower in the treatment group than the model group, the differences being significant (F=9.009--11.231,P〈0.05). At weeks 2 and 3, a comparison between the three groups in terms of levels of TGF-131, Smad2, FN and CTGF showed the highest was in the model group, and the lowest the sham operation group, the differences being significant (F=10.365-11. 775,P〈0.05). Conclusion Atorvastatin can significantly relieve renal interstitial demage and fibrosis caused by unilateral ureter obstruction, which is associated with the inhibition of TGF-β1 expression.
