膀胱全切除并移植肾输尿管皮肤造口术二例的护理

摘　　要：目的探讨膀胱全切除并移植肾输尿管皮肤造口术的护理。方法回顾性总结2例同种异体肾移植术后患者行膀胱全切除并移植肾输尿管皮肤造口术的临床资料及围术期的护理体会。结果 2例患者的造口均位于右侧下腹,留置输尿管支架管接引流袋,手术成功,未出现并发症。出院后6个月,2例患者移植肾功能良好,输尿管造口功能正常,日常生活均能自理。结论术前重点做好患者的心理护理及选择合适的造口产品,术后做好引流管护理、抗感染护理、排斥反应的治疗及护理、皮肤造口护理以及出院指导,可提高膀胱全切除并移植肾输尿管皮肤造口术的手术效果,改善患者的生活质量。Objective To explore the nursing care after total cystectomy transplanted kidney ureter skin ostomy.Methods Retrospectively summary of two patients＇ clinical data and nursing care of perioperative period after cystectomy transplanted kidney ureter skin ostomy.Results Two patients＇ stoma are located on the right lower abdomen,with indwelling ureteral stent and drainage bag,and the operation was successful with no complications.6 months after discharge,2 patients have good renal function and ureteral stoma function,and they are able to take care of themselves.Conclusion During the preoperative period,we should focus on psychological care of patients and choosing the right stoma product,while during the postoperative period we should do well in the care of drainage tube,anti-infection treatment,and treatment of rejection,skin and discharge guidance.It can improve the effect of cystectomy skin transplanted kidney ureter ostomy surgery and improve the quality of life of patients.
