Analysis on current situation of hospital acquired infections in tertiary hospital

摘　　要：目的通过对三甲医院医院感染现状的调查和分析,有针对性的提出干预措施,以期降低医院感染发生率。方法对2011年7月-2012年6月医院各科室感染例数、感染部位和感染病原菌进行统计描述。结果调查医院住院患者33 250例,发生医院感染602例,感染率为1.81%,内科感染率最高的科室为内分泌科和血液肿瘤科,分别为4.53%和2.90%,外科感染率最高的科室为烧伤科和胸外科分别为5.17%和5.08%;呼吸道是主要的感染部位占76.07%;检出病原菌革兰阴性菌为主占73.80%,病原菌检出前3为依次为肺炎克雷伯菌、大肠埃希菌和鲍氏不动杆菌,分别占22.29%、12.05%及11.45%。结论应增加医院感染管理力度,重点控制科室感染,减少侵入性操作和抗菌药物滥用,完善病房管理制度和探视制度,加强健康教育干预,以降低医院感染。OBJECTIVE To investigate and analysis the current situation of hospital acquired infections in tertiary hospital, so as to provide targeted interventions to decrease the infection rate. METHODS The cases of inpatients from Jul. 2011 to Jun. 2012 were reviewed to statistically desribe the number of infected patients, as well as the sites and strains of infection. RESULTS During the research, 602 cases of 33250 inpatients suffered hospital acquired infections. The prevalence rate was 1.81 %, and endocrinology department got the highest rate（4.53 %）, followed by hematology and oncology （2. 90%）, while the top two of surgery was burns and thoracic surgery, accounting for 5.17 % and 5.08 %. The main infection site was respiratory tract （76.07 %）. Gram-negative hacterium was the top pathogen , accounting for 73.8%. Klebsiella pnenmoniae, Escherichia coli ,and Acinetobacter baumannii were the top three main selies, accounting for 22. 29%, 12. 05% and 11. 45%, respectively. CONCLUSION Strengthening the nosocomial infection management, particularly controlling department infections, lessing invasive process, using antibacterial drug rationally, perfecting management of ward and visit, and providing educational activities should be carried out to reduce the rate of nosocomial infections.
