妊娠过程中孕妇体内sCD40及sCD40L水平变化与子痫前期发病及肾功能的相关研究

摘　　要：目的 探讨妊娠过程中孕妇体内sCD40及sCD40L水平变化与子痫前期发病和肾功能的相关性.方法 以我院2011-01~2013-01间收治的子痫前期孕妇78例为研究对象,并以同期健康孕妇50例为对照组,根据孕妇病情,将研究对象分为轻度子痫前期组（42例）、重度子痫前期组（36例）,比较3组孕妇体内sCD40和sCD40L的含量及肾功能指标的水平.结果 在3组孕妇sCD40、sCD40L、24 h蛋白尿、UA、Cr、BUN等含量的比较中,重度子痫前期高于轻度子痫前期,轻度子痫前期高于正常孕妇,在子痫前期孕妇中,sCD40和sCD40L与UA、24 h尿蛋白和CRP含量呈正相关关系.而在健康孕妇中,sCD40和sCD40L与正常孕妇肾功能各指标均呈现相关关系.结论 子痫前期孕妇体内sCD40和sCD40L的水平显著提高,并且其与体内多种肾功能监测指标存在相关性.Objective To discuss the relationship between variance levels of sCIM0 and sCIMOL in preeclamptic patients with renal damage in pregnant women in pregnancy process. Methods 78 patients were selected as the research objects. 42 pregnant women with mild preeclampsia were in the mild preeclampsia group, 36 pregnant women with severe preeclampsia were in the severe preeclampsia group. Sen40 and sCD40L in pregnant women were measured and compared among the three groups. Results In the comparison of CRP, UA, Cr, 24 hours urine protein excretion and the level of sCIM0 and sCD40L, the ones of the severe preeclampsia group were the highest in the three groups, and the ones of the mild preeclampsia group were higher than the ones of the control group. In the correlation analysis, sCIM0 and sCD40L have a positive correlation relationship with the renal function indices among the pregnant women with preeclampsia. However, there was no positive correlation between the sCD40 or sCD40L and the renal function indices in the healthy pregnant women. Conclusion The plasma concentrations of sCIM0 and sCD40L are significantly higher in the pregnant women with preeclampsia, which may be involved in the development of preeclampsia and contribute to the kidney damage.
