超敏C反应蛋白、降钙素原蛋白及高迁移率蛋白-1在小儿迁延性腹泻的表达及生物学意义

摘　　要：目的探讨超敏C反应蛋白（hs—CRP）、降钙素原蛋白（PCT）及高迁移率蛋白-1（HMGBl）在小儿迁延性腹泻中的表达及其生物学意义，为临床诊治提供新的依据。方法按照儿科学诊断标准选择2009年3月一2011年3月在我院治疗的迁延性腹泻患儿共236例，采用速率散射免疫比浊法、免疫化学发光法及ELISA方法检测血标本中的hs—CRP、PCT及HMGBl，同时送检血和便标本培养。判定hs—CRP〉lOmg／L、PCT〉0．5ug／L、HMGBI的OD值≥2．27为阳性，以P〈0．05差异有统计学意义。结果血和便培养阴性患儿223例（占94．49％），其中hs—CRP阳性107例，阳性率为47．98％；PCT阳性134例，阳性率为60．09％；HMGBl阳性211例，阳性率为94．62％；HMGBl与hs—CRP、PCT表达比较差异有统计学意义（P〈0．05）；hs—CRP和PCT比较差异无统计学意义。血和便培养阳性的患儿13例，hs—CRP阳性10例、PCT阳性9例、HMGBl阳性12例，三者表达差异无统计学意义（P〉0．05）。结论在慢性炎症发病过程中，HMGBl-的表达较PCT和hs—CRP更敏感，可能反映病情的严重程度，可作为临床慢性炎症治疗的指标。Objective To investigate the diagnosis value of high sensitive C -reactive protein （hs- CRP） / se- rum procaleitonin （PCT） and high mobility group box - 1, HMGB1 ） in ehlidren with Protracted diarrhea for clinical new treatment. Methods There had 236 patients in the study by clinical diagnosis during March 2009 - March 2011. hs - CRP was tested by rate of scattering the immune turbidimetD-, PCT was tested by immu- nochemilurninometrie assays and HMGBlwas tested by ELISA. At the same time makes the blood and culture specimens The positive standard are hs - CRP 〉 10rag/L, PCT 〉 0.5Ucl/L and OD ~〉2.27. Difference was statistically significant （P 〈 0. 05 ） . Results Blood and then develop negative children 223 cases （94. 49% ）, with 107 cases hs - CRP positive, positive rate is 47.98% ; 134 PCT positive, positive rate is 60. 09% ; 211 cases of HMGB1 positive rate is 94. 62% ; HMGB1 compared with hs - CRP and PCT expres- sion difference was statistically significant （ P 〈 0. 05 ） The hs CRP and PCT is no statistically significant difference. Blood and cultivate positive children 13 cases, 10 cases of hs - CRP positive, PCT 9 cases, HMGB1 12 cases, three expression differences have no statistical significance （P 〉 0. 05） . Conclusions In the process of chronic inflammation, the expression of HMGB1 - was the hs - CRP and PCT and more sen- sitive, may reflect the severity of the illness, can be used as an index of clinical treatment of chronic inflam- mation.
