摘 要:目的研究防己茯苓汤防治急性肾损伤(AKI)的可能作用机理。方法 36只Wistar大鼠随机分成正常组、模型组、防己茯苓汤组,每组12只。模型组和防己茯苓汤组经腹腔注入脂多糖5mg/kg造成大鼠AKI模型,防己茯苓汤组于注射前7天开始,灌胃防己茯苓汤18.375g/(kg·d),每次间隔12h;正常组、模型组在相应时间点灌等量生理盐水。统计各组大鼠腹腔注射后24h内的死亡情况,计算死亡率。分别于注射后12h、24h检测血清肌酐(SCr)、尿素氮(SUN)含量,尿量及尿N-乙酰-β-氨基葡萄糖苷酶(NAG酶),肾组织肾损伤分子-1(KIM-1)、中性粒细胞明胶酶相关脂质运载蛋白(NGAL)mRNA及蛋白的表达。结果模型组大鼠在注射后24h死亡率为53.3%、防己茯苓汤组为41.7%、正常组无死亡发生,防己茯苓汤组死亡率显著低于模型组(P<0.05)。防己茯苓汤组在24h时SCr、BUN、尿量、尿NAG酶、KIM-1mRNA及其蛋白、NGAL mRNA及其蛋白表达较模型组同时间点有所下降(P<0.05)。结论防己茯苓汤具有防治AKI的作用,可能与抑制KIM-1和NGAL蛋白及其mRNA表达作用有关。Objective To study the possible mechanism of Fangji Fuling Decoction [ decoction of Fangji (Radix Stephaniae Tetrandrae, 防己) and Fuling (Poria, 茯苓),防己茯苓汤] for acute kidney injury (AKI). Methods Totally 39 Wistar rats were randomized into the normal group, model group and Fangji Fuling Decoction group, with 13 in each. The AKI rat models were established by intraperitoneal injection of 5 mg/kg lipopolysaccharide (LPS) in the model group and Fangji Fuling Decoction group. The Fangji Fuling Decoction group was gavaged with 18. 375 g/ (kg · d) Fangji Fuling Decoction twice daily from 7 days before injection. The normal group and model group were gavaged with equal volume of normal saline at the corresponding time points. The mortality within 24 h after intraper- itoneal injection was calculated. The contents of serum creatinine (SCr) and blood urea nitrogen (BUN), urine volume and the expressions of urinary N-acetyl-β-glucosaminidase (NAG), kidney injury molecule-1 (KIM-1), KIM-1 mRNA, neutrophil gelatinase-associated lipocalin (NGAL), and NGAL mRNA were detected. Results The mortality within 24 h after injection in the model group and Fangji Fuling Decoction group was 53.8% and 38.5% respectively. There was no death in the normal group. The mortality in the Fangji Fuling Decoction group was signifi- cantly lower than that in the model group (P 〈 0. 05). The contents of SCr and BUN, urine volume and the expressions of NAG, KIM-1, KIM-lmRNA, NGAL and NGAL mRNA in the Fangji Fuling Decoction group was significantly lower than those in the model group ( P 〈 0.05). Conclusion Fangji Fuling Decoction is effective for AKI, which may be relevant to inhibiting the expressions of KIM-1, KIM-1 mRNA, NGAL and NGAL mRNA.
