摘 要:目的探讨回阳生肌散促进糖尿病皮肤溃疡大鼠创面愈合的可能作用机制。方法 56只SD大鼠随机分为对照组8只、模型组12只、全方组12只、温阳组12只和活血组12只。除对照组外其余各组采用"腹腔注射链脲佐菌素(STZ)-激素干预-皮肤缺损-塑料环埋置"方法制备糖尿病皮肤溃疡动物模型。对照组和模型组给予黄凡士林纱条,全方组给予回阳生肌散全方药纱条,将回阳生肌散方拆方为温阳益气方和活血生肌方,温阳组给予温阳益气方药纱条,活血组给予活血生肌方药纱条。各组每天给药1次,15天后检测创面新生肉芽组织中血管内皮生长因子(VEGF)、血管内皮细胞生长因子受体-2(VEGFR-2)、内皮型一氧化氮合酶(eNOS)的表达水平。结果模型组肉芽组织中VEGF、VEGFR-2、eNOS表达水平均明显低于对照组(P<0.01),全方组VEGF表达水平明显高于模型组和活血组(P<0.01),全方组VEGFR-2、eNOS表达水平明显高于模型组、温阳组和活血组(P<0.01)。结论回阳生肌散可能通过提高创面肉芽组织中VEGF、VEGFR-2、eNOS的表达水平,促进创面愈合。Objective To study the possible mechanism of Huiyang Shensi Powder ( powder to return yang and engender flesh,回阳生肌散) on angiogenesis-related factors in rat with diabetic skin ulcer. Methods Fifty-six SD rats were randomized into the control group (n = 8 ), model group (n = 12) , whole formula group (n = 12 ), Wenyang group (n = 12) and Huoxue group (n = 12). The rat models of diabetic skin ulcer were established by intraperitoneal injection of streptozotocin (STZ) , hormone intervention, artificial skin defects and stimulation of plastic ring in all groups except the control group. The control group and model group were given petrolatum gauze. Huiyang Shengfi Powder was disassembled into Wenyang Yiqi Formula (Yang-Warming Qi-Boosting Formula,温阳益气方) and Huoxue Shengfi Formula (Blood-Quickening Flesh-Engendering Formula,活血生肌方). The whole formula group, Wenyang group and Huoxue group were given ointment gauze with Huiyang Shengfi Powder, Wenyang Yiqi Formula and Huoxue Shengii Formula respectively. Each group was treated once daily for 15 days. The expressions of vascular endothelial growth factor (VEGF), vascular endothelial growth factor receptor-2 (VEGFR-2) and endothelial nitric oxide synthase (eNOS) in granulation tissue were detected. Results The expression levels of VEGF, VEGFR-2 and eNOS in the model group were significantly lower than those in the control group (P 〈 0.01 ). The expression level of VEGF in the whole formula group was significantly higher than that in the Huoxue group and model group (P 〈 0. 01 ). The expression levels of VEGFR-2 and eNOS in the whole formula group were significantly higher than those in the Wenyang group, Huoxue group and model group (P 〈 0. 01 ). Conclusion Huiyang Shengji Powder can promote wound healing by increasing the expression levels of VEGF, VEGFR-2 and eNOS in granulation tissue.
