先天性睑裂狭小综合征一期手术疗效分析

摘　　要：目的 探讨一期手术矫正先天性睑裂综合征的手术技巧和临床疗效,为今后临床应用提供理论依据.方法 2009年5月至2012年1月,采用内眦和外眦成形术联合上睑下垂矫正术,治疗先天性睑裂狭小综合征患者20例.术后随访6～24个月,观察睑裂横径,睑裂纵径,内眦间距,上睑动度等参数变化.结果 患者一次性接受内眦和外眦成形术联合上睑下垂矫正术,术后患者内眦赘皮消失,睑裂横径由术前的（18.1±1.1）mm增加到（29.3±1.2）mm,睑裂纵径由术前的（3.2±0.9）mm增加到（8.1±1.2）mm,内眦间距由术前的（39.0±1.3）mm减少到（32.4±1.4）mm,上睑动度由术前的（2.1±0.9）mm增加到（6.5±1.1）mm,与术前相比差异均具有统计学意义（P＜0.05）.重睑线自然且美观.术后随访6～24个月,无睑外翻、睑内翻及暴露性角膜炎等并发症发生,效果良好.结论 同期实施内眦和外眦成形术联合上睑下垂矫正术治疗先天性睑裂狭小综合征,能够达到良好疗效,并且缩短了手术周期.Objective To explore the skill and efficacy of one-stage operation for congenital blepharophimosis syndrome, and to provide a theoretical basis for future clinical application. Methods From May 2009 to January 2012, 20 patients with congenital blepharophimosis syndrome were treated with the medial and lateral canthoplasty combined with blepharoptosis correction. Patients were followed up for 6-24 months to observe the changes in transverse dimension, vertical dimension, inner canthal distance and upper eyelid movement. Results The epicanthal fold disappeared in all patients after operation. Transverse dimension, vertical dimension, inner canthal distance and upper eyelid movement were significantly improved from preoperative （18.1±1.1） mm, （3.2±0.9） mm, （39.0±1.3） mm and （2.1± 0.9） mm to postoperative （29.3±1.2） mm, （8.1±1.2） mm, （32.4±1.4） mm and （6.5±1.1） mm, respectively （P〈0.05）. The double eyelid was natural and beautiful. After 6-24 months of follow-up, no patients had ectropion, entropion, exposure keratitis, and other complications. Conclusion The medial and lateral canthoplasty combined with blepharoptosis correction results in a good outcome with the shortening of operation cycle in the treatment of blepharophimosis syndrome.
