摘 要:目的观察依达拉奉对胸腔镜单肺通气患者围手术期炎性因子表达和氧化应激的影响。方法选择37例术中实施胸腔镜单肺通气的手术患者,麻醉后随机分为2组:①对照组(n=18):在气管插管后给予生理盐水100 mL静脉滴注,30 min内滴完;②依达拉奉组(n=19):在气管插管完成后给予依达拉奉0.5 mg/kg溶解于100 mL生理盐水内静脉滴注,30 min内滴完。观察2组患者围手术期动脉血气变化,白细胞介素8(IL8)、白细胞介素10(IL10)血浆浓度、超氧化物歧化酶(SOD)、丙二醛(MDA)和中性粒细胞数目(PMN)检测。结果和气管插管后相比,2组患者的动脉血氧分压在单肺通气时均有下降(P<0.05)。和手术开始相比,2组患者血浆IL8浓度在单肺通气1 h、手术结束及手术后6 h均升高(P<0.05);和对照组相比,依达拉奉组IL8浓度在单肺通气1 h、手术结束及手术后6 h均降低(P<0.05);和手术开始相比,2组患者血浆IL10浓度在单肺通气1h、手术结束及手术后6 h均升高(P<0.05),和对照组相比,依达拉奉组IL10浓度在单肺通气1 h、手术结束、手术后6 h及术后24 h均升高(P<0.05)。依达拉奉组SOD含量在单肺通气1 h、手术结束、术后6 h及术后24 h均高于对照组(P<0.05),而MDA和PMN低于对照组(P<0.05)。结论依达拉奉具有抑制单肺通气机体IL8表达、促进IL10表达和清除氧自由基的作用。【Objective】 To observe the effect of edaravone on expression of inflammatory factors and oxidativestress in patients underwent thoracoscope one lung ventilation during perioperative period. 【Methods】37patients underwent selected thoracoscope operations with one lung ventilation were randomly divided into two groups after anesthesia. The control group(n=18) was received saline 100 mL intravenous drop in 30 min; The edaravone group(n=19) was received edaravone 0.5 mg/Kg with saline 100 mL intravenous drop in 30 min. Variance of blood gas, and the serum concentration of interleukin-8(IL8), interleukin-10(IL10), superoxide dismutase(SOD),malonaldehyde(MDA), and the number of neutrophils(PMN) during preoperative period were observed.【Results】Compared with trachea cannula, the PaO2of patients from both two groups were decreased during one lung ventilation(P0.05). The serum concentration of IL8 of both two groups patients were increased after one lung ventilation 1 h,surgery ends and postoperative 6 h as compared with surgery began(P 0.05), but were lower in the edaravone group than in the control group(P 0.05). The serum concentration of IL10 of patients from both two groups was increased after one lung ventilation 1 h, surgery ended and postoperative 6 h as compared with surgery began(P 0.05), but was higher in the edaravone group than in the control group(P 0.05). The serum concentration of SOD was higher,and the serum concentration of MDA and PMN was lower in the edaravone group than in the control group after one lung ventilation 1 h, surgery ends, postoperative 6 h and 24 h as compared with surgery began(P 0.05). 【Conclusion】Edaravone can effectively inhibit the expression of inflammatory factors and oxidativestress in patients underwent one lung ventilation during preoperative period.
