温阳益气、活血通脉方治疗冠心病随机平行对照研究

摘　　要：[目的]观察温阳益气、活血通脉方治疗冠心病疗效。[方法]使用随机平行对照方法，将76例住院患者按随机数字表法分为两组。对照组38例硝酸酯类：硝酸甘油片，舌下含服，0．5mg／次，每5min可重复1片，直至疼痛缓解，如果15min内总量达34后疼痛持续存在，应立即就医。硝酸异山梨酯片（消心痛），5mg／次，2次／d，舌下给药。他汀类降血脂药物：阿托伐他汀钙片（立普妥），10mg／次，1次／d，晚餐时服用。或洛伐他汀片，10mg/次，1次／d，晚餐时服用。抗血小板药物：阿司匹林肠溶胶囊，口服，75～300mg／次，1次／d；冠状动脉痉挛病人首选钙通道阻滞剂：盐酸地尔硫卓片，30～60mg／次，1～2次／d，口服。治疗组38例温阳益气、活血通脉方（党参15g，麦冬12g，五味子8g，瓜蒌皮15g，桂枝8g，丹参、川芎、赤芍、莪术各15g，红花10g）；1剂／d，水煎200mL，早晚口服。连续治疗14d为1疗程。观测临床症状、不良反应。治疗1疗程，判定疗效。[结果]治疗组痊愈11例，有效18例，无效7例，恶化2例，痊愈率28．95％，总有效率94．74％。对照组痊愈9例，有效19例，无效5例，恶化5例，痊愈率23．68％，总有效率86．84％。临床疗效治疗组优于对照组（P〈0．05）。[结论]温阳益气、活血通脉方治疗冠心病效果显著，值得推广。[ Objective ] To observe the yang qi, Huoxuetongmai Method effect of the treatment of coronary heart disease . [ Methods ] Random parallel control method, 76 cases of hospitalized patients were randomly divided into two groups. The control group 38 cases nitrates ： nitroglycerin tablets , sublingual , 0.5mg / times , every five minutes a repeat until pain relief , pain persists , if the total amount of up to three after 15min , should seek immediate medical attention . Or isosorbide mononitrate tablets （ isosorbide dinitrate ）, 5mg / times, 2 times / d, sublingual administration. He statins ： atorvastatin calcium （ Lipitor ）, 10mg / times, 1 times / d, while the evening meal . Or lovastatin tablets , 10mg / times, 1 times / d, while the evening meal . Antiplatelet drugs ： aspirin enteric-coated capsules , oral , 75 - 300mg / times, 1 times / d; coronary spasm patients preferred calcium channel blockers ： Dihiazem Hydrochloride tablets , 30 - 60mg / times , 1 to 2 times / d, orally. Treated 38 cases of yang qi , Huoxuetongmai Method （ Dangshen 15g, Maidong12g, Wuweizi 8g, Guaweipi 15g, Guizhi 8g, Danshen, Chuanxiong, Chishao, Eshu each 15g, Honghua 10g） ; l / d, decoction 200mL, sooner or later orally. 14d is a continuous course of treatment . Observing clinical symptoms of adverse reactions. A course of treatment to determine efficacy. [ Results ] The cure in 11 cases, effective in 18 cases, seven cases , the deterioration of two cases , the cure rate is 28.95% , the total efficiency of 94.74%. The control group recovered nine cases , effective in 19 cases, five cases , the deterioration in 5 cases, the cure rate 23.68% , 86.84% total efficiency . The clinical efficacy of the treatment group than the control group （ P〈0.05 ）. [ Conclusion ] Yang qi , Huoxuetongmai Method effect of the treatment of coronary heart disease significantly , should be introduced.
