辨证分型汤剂内服与针刺联合西药治疗急性缺血型脑梗塞随机平行对照研究

摘　　要：[目的]观察中西医结合治疗急性缺血型脑梗塞疗效。[方法]使用随机平行对照方法,将120例住院患者按随机数字表法随机分为两组。对照组60例阿司匹林肠溶片0.2g/次,1次/d,晨起口服;小牛血清去蛋白提取物注射液20mL+250mL5%葡萄糖(糖尿病加氯化钠),静脉缓慢滴注,1次/d;酌情予吸氧及抗凝等处理。治疗组60例辨证治疗:风痰入络,真方白丸子加减;风阳上扰,天麻钩藤饮加减;阴虚风动,镇肝熄风汤加减;配合针刺治疗,1次/d,取穴:太阳(双),合谷,百会,三阴交(双),足三里(双),太冲(双);眼针:脾区,肾区,上焦区;头针:运动区,语言区;西药治疗同对照组。两组均连续治疗2周为1疗程,观测临床症状、不良反应。治疗1疗程,判定疗效。[结果]治疗组基本治愈11例,有效34例,进步13例,未愈3例,总有效率95.00%。对照组基本治愈8例,有效24例,进步16例,未愈12例,总有效率80.00%,治疗组疗效优于对照组(P<0.05)。两组均无不良反应。[结论]中西医结合治疗急性缺血型脑梗塞效果显著,值得推广。[ Objective ] To observe the curative effect of treatment of acute ischemic cerebral infarction with combination of traditional Chinese medicine and western medicine. [ Methods ] Using random parallel control method, 120 cases of hospitalized patients were randomly divided into two groups according to random number table method. Control group： 60 cases taking enteric-coated aspirin, 0.2 g/times, 1/d, since morning; The calf serum injection to protein extract 20 ml, added in 250 ml of 15 % glucose injection （ diabetes in 250 ml of 0.9 % sodium chloride injection ）, slowly intravenous dripped, 1/d ; Discretion to oxygen and anticoagulant treatment. Treatment group： 60 cases with syndrome differentiation treatment：Fengtanruluo, Zhenfangbaiwanzi were added and subtracted; Fengyangshangkang, Tianmagoutengyin were added and subtracted;Yinxufengdong, Zhenganxifengtang were added and subtracted; Cooperate with acupuncture treatment, day l/d, find out ： Taiyang （ double ）, Hegu, Baihui, Sanyinjiao （ double ）, Zusanli （ double ）, Taichong （ double ） ; Eye needle： the spleen, kidney area, the Shangjiao area; Head needle： motor areas, language area. Both groups were treated 2 weeks for 1 course of treatment, observated the clinical symptoms and the presence of adverse reactions. After a course treatment, judged curative effect. [ Results ] The treatment group ： 11 cases were basic cured; 34 cases were effective, 13 cases were progressive, 3 cases were unchanged, total effective rate was 95.00%. The control group： 8 cases were basic cured; 24 cases were effective, 16 cases were progressive,12 cases were unchanged, total effective rate was 80.00%. The curative effect of treatment group is better than that of control group （ P〈 0.05 ） . Two groups had no adverse reactions. [ Conclusion ] the treatment of acute ischemic cerebral infarction with combination of traditional Chinese medicine and western medicine is significant, and worthy to promot to use.
