探讨加快盐水推注流率对消除头颈CTA颈部对比剂伪影的影响

摘　　要：目的:探讨消除对比剂伪影干扰颈部血管显示的方法。方法:将46例患者随机分为A、B 2组,扫描参数和重建参数均相同,A组23例,B组23例。使用双筒高压注射器,经肘正中静脉注入,2组病例的对比剂用量均为50 mL,流率5.0 mL/s,生理盐水50 mL。A组采用与对比剂相同的5.0 mL/s的流率推注冲管生理盐水;B组采用高于对比剂流率的方式,以6.0 mL/s的流率推注冲管生理盐水。结果:46例均能显示颈动脉、椎动脉及其主要分支,A组23例锁骨静脉残留对比剂的浓度普遍偏高。B组21例锁骨下静脉腔内存留的对比剂被稀释,不形成干扰伪影,效果良好;2例有对比剂残留,对比剂浓度已降低。结论:加快冲管盐水的推注能有效降低锁骨下静脉对比剂的残留浓度,达到淡化的效果,消除对比剂产生的伪影。Objective To eliminate the effect of the contrast artifact on displaying cervical vesseles. Methods There were 46 patients divided into Group A and Group B equally, with constant scanning parameters and reconstruction parameters. Binoculors pressure syringe was used to inject the agent and contrast through median cubital vein. The contrasts in the two groups had their dosage as 50 mL, flow rate as 5.0 mL/s and the saline dosage as 50 mL. The agent in Group A had the same flow rate as that of the contrast, and the one in Group B bad its flow rate as 6.0 mL/s. Results All the cases had their carotid arteries, vertebral arteries and their branches displayed clearly. The 23 ones in Group A had the concentration of the contrast remained in the subclavian vein increased, while 21 ones in Group B had the contrast diluted and no artifact appeared, and the remained 2 ones had the concentration decreased. Conclusion The increased flow rate of the injected saline can decrease the concentration of the remained contrast, and thus the artifact can be eliminated.
