摘 要:目的 研究原发性失眠症患者抑郁状态和皮肤交感反应的改变,以探讨此类患者的临床诊疗指标.方法 选取符合入组标准的原发性失眠症患者102例,对照组为同期健康体检者52例,2组患者均行汉密顿抑郁量表(HAMD)和皮肤交感反应(SSR)检测.结果 原发性失眠组患者HAMD总分(15.54±5.56)分,对照组(8.23±4.01)分,2组比较有显著性差异(P<0.01).失眠患者中女性患者占失眠总人数的70%,比例明显高于男性;女性患者的HAMD评分(17.17±5.33)分,男性患者(13.13±5.08)分,2者比较有显著性差异(P<0.01).原发性失眠患者中HAMD> 17分的SSR潜伏期比HAMD≤17分者显著延长(P<0.05),二者均比对照组潜伏期显著延长(P<0.01);原发性失眠患者中HAMD>17分的SSR波幅上肢与HAMD≤17分者比较无显著性差异(P>0.05),下肢有显著性差异(P<0.05);二者均比对照组波幅显著减低(P均<0.01).原发性失眠患者的SSR潜伏期与HAMD评分具有正相关性(r=0.404~0.486,P<0.01);而下肢SSR波幅与HAMD评分具有负相关性(r=-0.295,P<0.01).结论 原发性失眠患者抑郁状态明显增高,女性有失眠及抑郁的易感性.原发性失眠患者皮肤交感神经兴奋性减低,伴发抑郁程度重者变化更为明显.通过SSR检查可了解原发性失眠症患者的自主神经功能改变,对临床诊治具有较高价值.Objective It is to observe the depression and sympathetic skin response changes of primary insomnia patients,and investigate the clinical diagnose and treat index of these patients.Methods 102 cases with primary insomnia patients in Neurology clinic was selected,and healthy control group was 52 cases which selected from physical examination for the same period.Two groups were received Hamilton Depression Scale (HAMD) and the sympathetic skin response (SSR) testing.Results The HAMD total score was 15.54 ± 5.56 in primary insomnia group,and was 8.23 ± 4.01 in control group,there was significant difference between the both groups (P < 0.01).Female patients accounted for 70% of the total patients,which was significantly higher than men in insomnia group; the HAMD score was 17.17 ± 5.33 of female and 13.13 ± 5.08 of male,there was significant difference (P < 0.01).The SSR latency of primary insomnia patients with HAMD > 17 was markedly extended than that with HAMD ≤ 17 (P < 0.05),but they were markedly longer than control group (all P < 0.01) ; there was no significant difference in SSR amplitude of upper limb between HAMD > 17 and HAMD≤17 of primary insomnia patients (P >0.05),but was significant difference in lower limb (P < 0.05) ; they were markedly lower than control group (all P < 0.01).There was a positive correlation between the SSR latency and HAMD score in primary insomnia patients (r =0.404 to 0.486,P < 0.01) ; but there was a negative correlation between the amplitude and HAMD score in lower limb (r =-0.295,P < 0.01).Conclusion The depression levels of primary insomnia patients are significantly higher; women have susceptibility to insomnia and depression.The sympathetic skin response excitability in patients with insomnia was reduced,which is more obvious in patients with insomnia and severe depression.
