摘 要:目的:探讨门奇静脉断流术后的患者结合丹参注射液的应用对OFR系统的影响。方法:肝硬化门脉高压症(PHT)患者37例,肝功能均为A级;正常对照组10例,门奇静脉断流术组18例,门奇静脉断流术结合丹参注射液治疗组19例。术前、术后第3 d、10 d晨空腹抽静脉血应用黄嘌呤氧化酶法测定超氧化物歧化酶(SOD),硫代巴比妥酸(TBA)法测定丙二醛(MDA)。结果:肝硬化PHT患者术前血清SOD活性二组均较对照组显著降低(P<0.01),而MDA则显著升高(P<0.01)。术后3 d、10 d手术组和丹参治疗组SOD活性仍明显低于对照组(P<0.01),而与术前组比术后10 dSOD活性升高有意义(P<0.05)。术后3 d、10 d手术组和丹参治疗组MDA含量均高于对照组,术后10天的MDA含量与术前组比降低显著(P<0.05)。术后10天SOD活性、MDA含量变化率比术后3 d的大,丹参治疗组SOD活性、MDA含量变化率也较手术组明显。结论:SOD、MDA的变化在肝硬化PHT的形成过程中可能起重要作用;而门奇静脉断流术后治疗时间的延长上述各参数的变化趋于明显且门奇静脉断流术能逆转上述各参数的变化,而丹参注射液的联合应用又进一步降低MDA、升高SOD,随着丹参治疗时间的延长各参数的变化率也明显加大。Objective: To study the effect of oxygen- derived free radicals (OFR) on esophagogastric devascularization patients treated with Danshen Injection. Methods: Among 37 cases of cirrhotic portal hypertension (PHT) patients,all pa- tients' liver function was A grade. The normal control group had 10 cases;esophagogastric devascularization group 18 ca- ses ;esophagogastric devascularization combined with Danshen Injection 19 cases. Before surgery, three days and ten days after treatment, morning fasting venous blood was taken out by xanthine oxidase method for determination of superoxide dis- mutase (SOD) and sulfur barbituric acid (TBA) method for the determination of malondialdehyde (MDA). Results: In patients with liver cirrhosis, PHT preoperative serum SOD activity in two groups was significantly lower than that of the con- trol group( P 〈0.01 ) ,while MDA increased significantly ( P 〈0.01 ). 3 and 10 days later,the activity of SOD in esopha- gogastric devascularization combined with Danshen Injection group was significantly lower than that of the control group ( P 〈0.01) ,and the elevated SOD activity had significance( P 〈 0.01 ) in preoperative group than that after 10 days( P 〈 0.05). 3 and 10 days after surgery,MDA in esophagogastric devascularization combined with Danshen Injection group was higher than that in control group. MDA educed significantly 10 days after surgery ( P 〈0.05). 10 days after surgery, SOD activity and MDA content change rate were obviously changed than those 3 days after surgery of esophagogastric devas- cularization combined with Danshen Injection group. Conclusion: The changes of SOD and MDA may play an important role in the formation process of PHT in liver cirrhosis and portal azygous disconnection. The treatment of postoperative time extension of the parameter changes tend to be obvious and esophagogastric devascularization can reverse the changes of each parameter,and combined application of Danshen Injection Injectio
